摘要
目的 分析中药湿热敷联合穴位按摩在中重度乳腺癌术后上肢淋巴水肿患者中的应用效果。方法选取2019年3月至2021年12月于上海中医药大学附属第七人民医院接受综合消肿治疗的80例中重度乳腺癌术后上肢淋巴水肿患者作为研究对象。采用随机数字表法将其分为观察组与对照组每组各40例。对照组给予综合消肿治疗,观察组在对照组基础上增加中药湿热敷联合穴位按摩,共干预28 d。比较两组患者的患肢臂围周径、上肢功能活动度及乳腺癌淋巴水肿症状指数。结果 观察组患者的患肢臂围周径干预前后差值、干预后上肢功能活动度评分均低于对照组(P<0.05)。干预前,两组患者乳腺癌淋巴水肿症状指数两部分及其维度评分比较,差异均无统计学意义(P>0.05)。干预后,两组患者在乳腺癌淋巴水肿的症状个数、症状严重程度、症状困扰方面比较,差异有统计学意义(P<0.05),观察组优于对照组;其中,症状困扰的功能、社交、情绪及归属感维度得分比较,差异均有统计学意义(P<0.05),但性生活、睡眠维度得分比较,差异无统计学意义(P>0.05)。结论 通过中药湿热敷联合穴位按摩对中重度乳腺癌术后上肢淋巴水肿患者进行干预,可以有效缓解患者患肢肿胀、增加上肢功能活动度及减轻相关症状。
Objective To analyze the effect of TCM wet⁃hot compress+acupoint massage in patients with upper extremity lymphedema after moderate⁃severe breast cancer surgery.Methods From March 2019 to December 2021,80 patients with upper extremity lymphedema after moderate⁃severe breast cancer surgery who received complex decongestive therapy at the Seventh People's Hospital Affiliated to Shanghai University of Traditional Chinese Medicine were selected.They were randomly divided into the observation group(n=40)and the control group(n=40)by using a random number table.The control group was given complex decongestive therapy,while the observation group was given TCM wet⁃hot compress+acupoint massage on this basis,for a total of 28 days.The arm circumference of the affected extremity,upper extremity range of motion and the score of Breast Cancer and Lymphedema Symptom Experience Index(BCLE⁃SEI)were compared between the two groups.Results The difference in arm circumference before and after intervention and the score of upper limb functional activity after intervention in the observation group were lower than those in the control group(P<0.05).Before the intervention,there was no significant difference between the two groups in the scores of the two parts and each dimension of BCLE⁃SEI(P>0.05).After the intervention,there were significant differences between the two groups in the number of symptoms,severity of symptoms and symptom distress(P<0.05),the observation group is superior to the control group.As for the symptom distress,there were significant differences in the scores of functional,social,emotional and sense of belonging dimensions(P<0.05),but there was no significant difference in the scores of sexual life and sleep quality dimensions(P>0.05).Conclusion Intervention with TCM wet⁃hot compress+acupoint massage can relieve the swelling of affected extremities,increase upper extremity range of motion and alleviate related symptoms in patients with upper extremity lymphedema after moderate⁃severe breast cancer surgery.
