摘要
目的探讨带蒂眶隔内脂肪瓣的牢固内固定在泪沟型眼袋畸形矫正术中的应用效果。方法随机选取厦门大学附属第一医院思明分院2020年1月—2021年12月收治的50例泪沟型眼袋畸形患者,实施带蒂眶隔内脂肪瓣的牢固内固定术治疗纳入研究组,另选取既往接受结膜入路上颌前间隙眶隔脂肪重置矫正术治疗的泪沟型眼袋畸形患者50例纳入对照组。比较两组患者手术情况,术前、术后眼袋膨出、泪沟、下眼睑皮肤松弛、色素沉着评分,随访术后不适症状分级、术后切口愈合时间、术后随访满意度评分。结果研究组手术时间、术中出血量、手术切口长度优于对照组,差异无统计学意义(P>0.05)。术后研究组眼袋膨出(0.46±0.12)分、泪沟(0.33±0.26)分、下眼睑皮肤松弛(0.43±0.37)分、色素沉着评分(0.48±0.13)分明显低于对照组(0.94±0.22)分、(0.77±0.32)分、(0.86±0.48)分、(0.98±0.21)分,差异有统计学意义(t=13.544、7.546、5.017、14.315,P<0.05)。两组患者术后不适症状分级、术后切口愈合时间比较,差异无统计学意义(P>0.05)。研究组术后随访满意度评分高于对照组,差异有统计学意义(P<0.05)。结论在泪沟型眼袋的畸形矫正术治疗上,接受带蒂眶隔内脂肪瓣的牢固内固定术治疗可以明显减轻并改善患者的眼袋膨出、泪沟、下眼睑皮肤松弛、色素沉着程度,提高患者的满意度,效果理想。
Objective To discuss and analyze the application effect of rigid internal fixation with the pedicled orbital septal fat flap in the correction of lacrimal pouch deformity.Methods 50 patients with lacrimal sulcus type eye bag de-formities admitted to the Siming Branch of the First Affiliated Hospital of Xiamen University from January 2020 to De-cember 2021 were randomly selected as the study group to undergo rigid internal fixation with a pedicle orbital fat flap,another control group was 50 cases of lacrimal sulcus type eye bag deformities who had previously undergone conjunctival approach to correct orbital fat in the anterior maxillary space.The surgical conditions,preoperative and postoperative scores of pouch bulge,lacrimal furrows,skin relaxation of lower eyelid and pigmentation were compared between the two groups.Postoperative discomfort symptom grading,postoperative incision healing time and postopera-tive satisfaction score were followed up.Results The operation time,intraoperative blood loss,and the incision length in the study group was better than that of the control group,and the difference was not statistically significant(P>0.05).The score of pouch bulge(0.46±0.12)points,lacrimal groove(0.33±0.26)points,lower eyelid skin relaxation(0.43±0.37)points and pigmentation(0.48±0.13)points in the study group were significantly lower than those in the control group(0.94±0.22)points,(0.77±0.32)points,(0.86±0.48)points and(0.98±0.21)points,the difference was sta-tistically significant(t=13.544,7.546,5.017,14.315,P<0.05).There was no statistically significant difference in post-operative discomfort symptom grade and postoperative wound healing time between the two groups(P>0.05).Follow-up satisfaction score of the study group was higher than that of the control group,and the difference was statistically signifi-cant(P<0.05).Conclusion In the treatment of deformities of lacrimal sulcus type eye bags,receiving solid internal fixa-tion with pedicle orbital fat flap can significantly reduce and improve the degree of eye bag swelling,lacrimal sulcus,lower eyelid skin relaxation,and pigmentation in patients,improve patient satisfaction,and achieve ideal results.
