摘要
目的:探究不同类型血液透析器用于维持性血液透析的临床效果,为临床医师选择合理透析方案提供参考。方法:选取2019年1月~2020年6月在中一东北国际医院接受维持性血液透析的慢性肾衰竭患者104例作为研究对象,依据随机数字表法,将研究对象分为A组、B组、C组和D组(每组26例)。A组应用低通量醋酸纤维素膜(CA-LF),B组应用高通量醋酸纤维素膜(CA-HF),C组应用低通量聚砜膜(PS-LF),D组应用高通量聚砜膜(PS-HF),对比四组患者透析前后炎症因子水平、主观营养评分(MQSGA)。结果:治疗后,四组患者IL-6水平、MQSGA评分均有所下降,且B、D组低于A、C组,D组低于B组;IL-10水平有所上升,且B、D组高于A、C组,D组高于B组(P<0.05)。结论:维持性血液透析患者微炎症、营养不良状况较为普遍,而高通量血液透析器对维持性血液透析患者微炎症、营养不良状况具备一定的改善作用。
Objective:To explore the clinical effect of different types of hemodialyzers in maintenance hemodialysis,and provide reference for clinical physicians to choose reasonable dialysis protocol.Methods:A total of 104 patients with chronic renal failure who received maintenance hemodialysis during from January 2019 to June 2020 in Zhongyi Northeast International Hospital were selected as the research subjects.According to the random number table method,the subjects were divided into four groups:group A,group B,group C and group D(26 cases in each group).Low flux cellulose acetate membrane(CA-LF),high flux cellulose acetate membrane(CA-HF),low flux polysulfone membrane(PS-LF)and high flux polysulfone membrane(PS-HF)were used respectively in group A,B,C and D.The levels of inflammatory factors,MQSGA were compared between the four groups before and after dialysis.Results:After treatment,the levels of IL-6 and MQSGA scores of the four groups were decreased,and the levels of IL-6 and MQSGA scores in groups B and D were lower than those in groups A and C,group D were lower than those in group B;The levels of IL-10 of the four groups were increased,and the levels of IL-10 of groups B and D was higher than that of groups A and C,group D were higher than that in group B(P<0.05).Conclusion:The microinflammation and malnutrition are common in maintenance hemodialysis patients,and high flux dialyzer can improve the micro inflammation and malnutrition of maintenance hemodialysis patients.
