血清sCD163/sTWEAK比值与川崎病患儿冠状动脉病变程度的相关性

Correlation Between Serum sCD163/sTWEAK Ratio and the Degree of Coronary Artery Lesion in Children with Kawasaki Disease

摘要 目的 探讨川崎病患儿血清可溶性单核巨噬细胞血红蛋白清道夫受体(sCD163)、可溶性肿瘤坏死因子样凋亡弱诱导因子(sTWEAK)水平变化及sCD163/sTWEAK比值与患儿冠状动脉病变的相关性。方法 选择2019年6月至2022年10月在郑州大学附属洛阳中心医院收治的220例川崎病患儿作为观察对象,另选取同期健康体检儿童100例作为对照组,根据冠状动脉Z值分为冠状动脉正常组172例,冠状动脉病变组48例,其中小冠状动脉瘤(SCAAs)(2.50.05),冠状动脉正常组与冠状动脉病变组之间血小板(PLT)、C反应蛋白(CRP)水平差异无统计学意义(P>0.05);与对照组比较,冠状动脉正常组与冠状动脉病变组患儿血清sCD163、sTWEAK水平、sCD163/sTWEAK比值升高(P<0.05);与冠状动脉正常组患儿比较,冠状动脉病变组患儿血清sCD163、sTWEAK水平、sCD163/sTWEAK比值和冠状动脉内径升高(P<0.05);与SCAAs组比较,LCAAs组与GCAAs组患儿血清sCD163、sTWEAK水平、sCD163/sTWEAK比值升高(P<0.05);与LCAAs组比较,GCAAs组患儿血清sCD163、sTWEAK水平、sCD163/sTWEAK比值升高(P<0.05);相关性分析结果显示,冠状动脉病变组患儿sCD163/sTWEAK比值与冠状动脉Z值呈正相关性(r=0.624,P<0.05)。结论 川崎病患儿血清sCD163/sTWEAK比值升高,与患儿冠状动脉病变相关,可作为诊断川崎病患儿发生冠状动脉病变的标志物。 Objective To investigate the correlation between serum soluble mononuclear macrophage hemoglobin scavenger receptor(sCD163),soluble tumor necrosis factor-like weak inducer of apoptosis(sTWEAK)levels and sCD163/sTWEAK ratio in children with Kawasaki disease and coronary artery lesions.Methods A total of 220 children with Kawasaki disease admitted to Luoyang Central Hospital affiliated to Zhengzhou University from June 2019 to October 2022 were selected as the observation objects,and 100 children with healthy physical examination during the same period were selected as the control group.According to the coronary Z value,they were divided into 172 cases with normal coronary artery and 48 cases in coronary artery disease group,including 25 cases of small coronary aneurysms(SCAAs)(2.5<Z<5),and 25 cases of large coronary aneurysms(LCAAs)(5≤Z<10)15 cases,giant coronary aneurysm(GCAAs)(Z≥10)8 cases.The levels of sCD163 and sTWEAK in the children’s serum was detected by enzyme-linked immunosorbent assay,and the ratio of sCD163/sTWEAK was calculated.The correlation between sCD163/sTWEAK ratio and the diameter of coronary artery in the coronary artery disease group was analyzed by the Pearson method.Results There were significant differences in the levels of white blood cell count(WBC),albumin(ALB),coronary artery diameter between the control group,the normal coronary artery group and the coronary artery disease group(P<0.05).There were no statistically significant differences in age,gender,neutrophil ratio,glutamic-oxaloacetic transaminase(GOT),creatine kinase isoenzyme MB(CK-MB)level between control group,normal coronary artery group and coronary artery disease group(P>0.05),while there were no statistically significant differences in platelets(PLT)and C-reactive protein(CRP)levels between normal coronary artery group and coronary artery disease group(P>0.05).Compared with the control group,the serum levels of sCD163,sTWEAK and the ratio of sCD163/sTWEAK were higher in the normal coronary artery group and the coronary artery disease group(P<0.05).Compared with children in the normal coronary artery group,the serum sCD163,sTWEAK levels,sCD163/sTWEAK ratio and coronary artery diameter in the coronary artery disease group were increased(P<0.05).Compared with SCAAs group,serum sCD163,sTWEAK levels and sCD163/sTWEAK ratio of children in LCAAs group and GCAAs group were increased(P<0.05).Compared with the LCAAs group,the serum levels of sCD163,sTWEAK and the ratio of sCD163/sTWEAK in the GCAAs group were increased(P<0.05).The results of correlation analysis showed that the ratio of sCD163/sTWEAK in the coronary artery disease group was positively correlated with the inner diameter of the rcoronary artery(r=0.624,P<0.05).Conclusion The serum sCD163/sTWEAK ratio in children with Kawasaki disease is increased,which is related to coronary artery disease in children,and can be used as a marker for the diagnosis of coronary artery disease in children with Kawasaki disease.
