摘要
目的分析以在读医学硕博研究生为代表的青少年群体颈椎失稳发生的相关表现及其生活习惯。方法以问卷形式收集91例在读医学硕博研究生的基本信息、SF-36评分、NDI评分和生活习惯信息。对所有受试者进行颈椎正侧位及功能位X线片拍摄,进行两相邻椎体间的角度位移、水平位移及颈椎曲度的数据测量,并根据测量结果将受试者分为正常组和失稳组进行比较。对受试者的生活习惯与其颈椎曲度、最大椎体角度位移的相关性进行分析。结果91例中,颈椎失稳50例,C_(3-4)失稳13例、C_(4-5)30例、C_(5-6)7例。失稳组的健康调查简表在生理功能、生理职能、机体疼痛、一般健康、情感职能等五个方面评分均显著低于正常组(P<0.05);失稳组NDI评分显著高于正常组(P<0.05);失稳组每天使用手机时长、单次低头持续时长和使用电脑屏幕高度等数据均显著高于正常组(P<0.05)。所有受试者的单次低头持续时长和使用电脑屏幕高度均与其颈椎曲度呈负相关(r<0),与其最大椎体角度位移呈正相关(r>0)。结论以在读医学硕博研究生为代表的青少年群体颈椎失稳所引起的颈椎功能障碍,严重影响了其生理健康乃至心理健康,生活质量明显下降。避免长时间、连续性的低头姿势和适当调高电脑屏幕高度,能够有效预防颈椎失稳的发生。
Objective To analyze the related performance and living habits of cervical instability in adolescents represented by postgraduates with master’s degree in medicine.Methods The basic information,SF-36,NDI score and living habits of 91 postgraduates with master’s degree in medicine were collected in the form of questionnaires.The X-ray films of all subjects were taken for the positive,lateral and functional positions of the cervical vertebrae.And the angular displacement,horizontal displacement and cervical curvature between the two adjacent vertebrae were measured.According to the measurement results,the subjects were divided into normal cervical vertebra group and instability group.The correlations between the living habits of 91 subjects and the curvature of cervical vertebra and the maximum angular displacement of vertebral body were analyzed.Results A total of 50 patients with cervical instability were studied,among whom there were 13 cases of C_(3-4) instability,30 cases of C_(4-5) instability and 7 cases of C_(5-6) instability.The scores of physical function,role physical,bodily pain,general health and role emotional in the instability group were significantly lower than those in the normal group(P<0.05),the NDI score in the instability group was significantly higher than that in the normal group(P<0.05),and the time of using mobile phone every day,the duration of a single bow and the height of computer screen in the instability group were significantly higher than those in the normal group(P<0.05).The duration of a single bow and the height of the computer screen in 91 subjects were correlated with cervical curvature and maximum angular displacement of vertebral body(P<0.05),with negative correlation(r<0)and positive correlation(r>0),respectively.Conclusion The cervical vertebra dysfunction caused by cervical instability in adolescents represented by postgraduates with master’s degree in medicine seriously affects their physical health and even mental health.Avoiding long-term and continuous posture of lowering the head and properly raising the height of the computer screen can effectively prevent the occurrence of cervical instability.
