椎间孔腹侧成形技术治疗L3-4脱垂型椎间盘突出症34例

摘要 目的探讨椎间孔腹侧成形技术治疗L 3-4脱垂型椎间盘突出症的临床疗效及技术特点。方法回顾性分析自2019年1月~2020年10月采用椎间孔腹侧成形技术行椎间孔镜减压术治疗的34例L 3-4脱垂型腰椎间盘突出症患者的临床资料,随访时间12~24个月。记录患者术前、术后1周、术后3个月、末次随访的疼痛VAS评分、Oswestry功能障碍指数(Oswestry disability index,ODI)和JOA评分,采用改良Macnab标准评价临床疗效。结果术中无硬膜撕裂、神经损伤并发症,术后无感染、脑脊液漏、神经根功能损伤加重、椎间盘突出复发等并发症发生。与术前相比,患者术后1周、术后3个月、末次随访时的VAS评分、ODI指数、JOA评分均获得显著改善(P<0.05)。末次随访采用Macnab标准对疗效进行评价,优27例、良5例、可2例,优良率为94.12%。结论椎间孔腹侧成形技术行椎间孔镜减压术治疗L_(3-4)向下脱垂型椎间盘突出症,临床疗效满意,可保留上关节突关节复合体,有效减少并发症。
