摘要
【目的】采用数据挖掘方法探讨现代中医药文献中治疗血瘀型糖尿病足的处方用药规律。【方法】检索2011~2021年中国知网(CNKI)、维普中文科技期刊数据库(VIP)、万方(Wanfang Data)数据库、中国生物医学文献数据库(CBM)中有关内服中药方剂治疗血瘀型糖尿病足的临床研究文献。基于中医传承辅助平台(V2.5),统计分析治疗血瘀型糖尿病足方剂药物的用药频次、四气五味及归经分布,挖掘其药物间的关联规则及新方组合。【结果】共纳入文献128篇,获得治疗血瘀型糖尿病足的有效病例的方剂128首,涉及中药152味。用药频次≥20次的有17味中药,分别为黄芪、当归、川芎、赤芍、红花、牛膝、桃仁、地龙、丹参、甘草、桂枝、鸡血藤、金银花、玄参、生地黄、白芍、水蛭。药性以温、寒为主,平性药次之;药味以甘、苦为主,辛味药次之;药物归经以归肝、心、脾经为主。基于层次聚类分析演化得到核心药物组合20个、新方组合10个。【结论】治疗血瘀型糖尿病足的用药以益气活血、化瘀通络类药物为主,常温寒并用,治疗多从肝、心、脾论治。
Objective To explore the prescription and medication rules in modern traditional Chinese medicine(TCM)literature for the treatment of blood stasis type of diabetic foot by data mining methods.Methods The clinical research literature on the treatment of blood stasis type of diabetic foot with oral use of Chinese herbal prescriptions issued in the year of 2011 to 2021 was retrieved from the databases of CNKI,VIP,Wanfang Data and CBM.Based on Traditional Chinese Medicine Inheritance Support System(V2.5),the frequency of medication,four properties and five flavors and the meridian tropism of the herbs in the prescriptions for the treatment of blood stasis type of diabetic foot were statistically analyzed,and the association rules and new prescription combinations of the drugs were explored.Results A total of 128 articles were included for the analysis,and 128 prescriptions for the effective cases of blood stasis type of diabetic foot were obtained,involving 152 Chinese herbal medicines.There were 17 Chinese medicines with the frequency≥20 times,and the medicines were Radix Astragali,Angelicae Sinensis Radix,Chuanxiong Rhizoma,Paeoniae Radix Rubra,Carthami Flos,Achyranthis Bidentate Radix,Persicae Semen,Pheretima,Salviae Miltiorrhizae Radix,Glycyrrhizae Radix et Rhizoma,Cinnamomi Ramulus,Spatholobi Caulis,Lonicerae Japonicae Flos,Scrophulariae Radix,Rehmanniae Radix,Paeoniae Radix Alba and Hirudo.The medicinal nature of the drugs was predominated by being warm and cold,followed by being mild.The flavor of the drugs was predominated by being sweet and bitter,followed by being pungent.And the medicines usually had the meridian tropism of liver,heart and spleen meridians.After hierarchical clustering,20 core drug combinations and 10 new formula combinations were obtained.Conclusion The treatment of diabetic foot with blood stasis type is mainly based on the drugs with the actions of replenishing qi and activating blood,resolving blood stasis and unblocking collaterals,and the warm and cold medicines are often used simultaneously.The treatment of diabetic foot with blood stasis type is mostly from the perspective of the liver,heart and spleen.
