摘要
目的探究多层螺旋CT容积扫描及三维重建测定容积增长率对孤立性肺结节良恶性的诊断价值。方法回顾性分析2019年12月至2021年12月江苏省海安市中医院接诊的75例孤立性肺结节患者临床资料。患者均在术前进行2次多层螺旋CT容积扫描及三维重建检测。比较恶性组和良性组患者病变处血容量、灌注峰值时间、增强峰值、表面通透性和容积增长率,并将P<0.05的因素进行Logistic回归进行多因素分析和ROC曲线分析。分析多层螺旋CT容积扫描联合三维重建测定容积增长率的诊断效能。结果恶性组的血容量、增强峰值、表面通透性、容积增长率均高于良性组(P<0.05),恶性组的灌注峰值时间短于对照组(P<0.05)。病变处血容量、表面通透性和容积增长率是预测孤立性肺结节良恶性的独立危险因素(P<0.05)。以血容量>6.51 ml/100 g为诊断孤立性肺结节的分割点,灵敏度为87.10%,特异度为84.62%。以表面通透性>15.63 ml/100 g·min作为诊断孤立性肺结节的分割点,灵敏为93.55%,特异度为76.92%。以血容量>6.51 ml/100 g为诊断孤立性肺结节的分割点,AUC为0.814,灵敏度为96.77%,特异度为84.62%。联合诊断的准确度为93.33%,灵敏度为96.77%,特异度为76.92%,漏诊率为3.33%,误诊率为3.00%。结论多层螺旋CT容积扫描及三维重建测定容积增长率对孤立性肺结节良恶性的诊断具有重要价值,两者联用可以增加诊断的准确度,降低漏诊率。
Objective To explore the diagnostic value of multi-slice spiral CT volume scanning and three-dimensional reconstruction in the benign and malignant solitary pulmonary nodules.Methods The clinical data of 75 patients with solitary pulmonary nodules of Hai an Hospital of Traditional Chinese Medicine in Jiangsu Province from December 2019 to December 2021 were retrospectively analyzed.All patients underwent two times of multi-slice spiral CT volumetric scanning and three-dimensional reconstruction.The blood volume,perfusion peak time,enhancement peak value,surface permeability,and volume growth rate of the malignant group and the benign group were compared,and the factors with P<0.05 were subjected to Logistic regression for multivariate analysis and ROC curve analysis.To analyze the diagnostic efficacy of multi-slice spiral CT volume scanning combined with three-dimensional reconstruction in measuring volume growth rate.Results The blood volume,enhanced peak,surface permeability,and volume growth rate of the malignant group were higher than those of the benign group(P<0.05),and the perfusion peak time of the malignant group was shorter than that of the control group(P<0.05).The blood volume,surface permeability,and volume growth rate of the lesion were independent risk factors for predicting the benign and malignant solitary pulmonary nodules(P<0.05).Taking blood volume>6.51 ml/100 g as the segmentation point for diagnosis of solitary pulmonary nodules,the sensitivity was 87.10%,and the specificity was 84.62%.The sensitivity was 93.55%and the specificity was 76.92%when the surface permeability was>15.63 ml/100 g•min.Taking blood volume>6.51 ml/100 g as the segmentation point for diagnosis of solitary pulmonary nodules,the AUC was 0.814,the sensitivity was 96.77%,and the specificity was 84.62%.The accuracy of combined diagnosis was 93.33%,the sensitivity was 96.77%,the specificity was 76.92%,the missed diagnosis rate was 3.33%,and the misdiagnosis rate was 3.00%.Conclusions Multi-slice spiral CT volume scanning and three-dimensional reconstruction are of great value in the diagnosis of benign and malignant solitary pulmonary nodules,and their combination can increase the accuracy of diagnosis and reduce the rate of missed diagnosis.
