摘要
目的探索胃泌素释放肽前体(gastrin-releasing peptide precursor,ProGRP)、癌胚抗原(carcinoembryonic antigen,CEA)、神经元特异性烯醇化酶(neuron-specific enolase,NSE)、细胞角蛋白19片段(cytokeratin 19 fragment,CYFRA21-1)、糖类抗原(carbohydrate antigen125,CA125)和鳞状细胞癌抗原(Squamous cell carcinoma antigen,SCC)对不同病理类型肺癌的诊断价值及六项标志物联合检测的临床意义。方法选取肺癌患者281例(肺腺癌173例,肺鳞癌55例,小细胞肺癌53例)纳入观察组,选取同期健康体检人群120例纳入对照组。采集二组血液标本检测ProGRP、CEA、NSE、CYFRA21-1、CA125和SCC水平,比较不同病理类型患者检测指标的差异,评价上述标志物的单独及联合诊断效能。结果肺癌患者的血清肿瘤标志物ProGRP、CEA、NSE、CYFRA21-1、CA125和SCC水平均高于对照组(P<0.05)。CEA在肺腺癌患者的平均水平最高,CYFRA21-1和SCC在肺鳞癌患者的平均水平最高,ProGRP和NSE在小细胞肺癌患者的平均水平最高。肺腺癌中CEA阳性率最高(57.8%),肺鳞癌中CYFRA21-1阳性率最高(89.1%),小细胞肺癌ProGRP阳性率最高96.2%。血清ProGRP、CEA、NSE、CYFRA21-1、CA125和SCC联合检测对肺癌的最优诊断敏感性为88.3%,特异性为96.7%,AUC=0.976。结论ProGRP、CEA、NSE、CYFRA21-1、CA125和SCC单独检测对肺癌病理分型有一定临床应用价值,联合检测对肺癌的诊断效能较高,适用于肺癌的筛检。
Objective To discuss the diagnostic value of ProGRP(gastrin-releasing peptide precursor),CEA(carcinoembryonic antigen),NSE(neuron-specific enolase),CYFRA21-1(cytokeratin 19 fragment),CA125(carbohydrate antigen125)and SCC(Squamous cell carcinoma antigen)in different pathological types of lung cancer and combined determination of six markers.Methods 281 lung cancer cases included 173 lung adenocarcinoma(LUAD)cases,55 lung squamous cell carcinoma(tanLUSC)cases,53 small cell lung cancer(SCLC)cases were included in the observation group,and 120 cases of healthy people were included as control group.Blood samples of the two groups were collected to detect serum ProGRP,CEA,NSE,CYFRA21-1,CA125 and SCC levels.Compared the differences of detection indicators in patients with different pathological types,and evaluated the diagnostic efficacy.Results The serum tumor markers ProGRP,CEA,NSE,CYFRA21-1,CA125 and SCC in lung cancer patients were higher than those in control group(P<0.05).The mean levels of CEA were highest in LUAD patients,CYFRA21-1 and SCC were highest in LUSC patients,and ProGRP and NSE were highest in SCLC patients.The positive rate of CEA was the highest in LUAD(57.8%),CYFRA21-1 was the highest in LUSC(89.1%),and ProGRP was the highest in SCLC(96.2%).The combined determination of six markers showed the best diagnostic sensitivity(88.3%)and specificity(96.7%),with AUC=0.976.Conclusion The individual determination of ProGRP,CEA,NSE,CYFRA21-1,CA125 and SCC has a clinical value in distinguish the pathological types of lung cancer.The combined determination has a high diagnostic efficiency and is suitable for the screening of lung cancer.
