摘要
目的 分析小儿斜视治疗中改良Parks切口对患儿泪膜稳定性、屈光度的影响。方法 选取2020年1月至2022年9月我院收治的斜视患儿82例,按随机数字表法分为对照组(41例,常规切口直肌调整缝线术)与改良Parks组(41例,改良Parks切口直肌调整缝线术),比较治疗前及治疗后两组患儿泪膜稳定性相关指标、球镜及柱镜屈光度,记录术后并发症发生情况。结果 改良Parks组治疗总有效率显著高于对照组;术后7 d改良Parks组泪膜破裂时间(break up time, BUT)显著高于对照组,角膜荧光染色(fluorescein staining of cornea, FL)评分显著低于对照组;术后1个月改良Parks组球镜屈光度、柱镜屈光度显著低于对照组;改良Parks组结膜肿胀、感染、干眼症、不适等并发症总发生率显著低于对照组,差异均有统计学意义(P <0.05)。结论 改良Parks切口用于小儿斜视手术治疗中可减轻对患儿屈光度的影响,保持泪膜稳定性,降低并发症发生率,具有较高临床应用价值。
Objective:To analyze the effect of modified Parks incision on tear film stability and diopter in children with strabismus.Methods:A total of 82 children with strabismus who were admitted to our hospital from January 2020 to September 2022 were selected and divided into the control group(41 cases,conventional rectus muscle adjustment suture)and the modified Parks group(41 cases,modified Parks incision rectus muscle adjustment suture)according to the random number table method.,The curative effect,measure and compare the tear film stability-related indicators,spherical and cylindrical diopters of the two groups before and after treatment were evaluated,and the occurrence of postoperative complications were recorded.Results:The total effective rate of the modified Parks group was significantly higher than that of the control group(P<0.05);the tear secretion test I(SⅠt)and tear film breakup time(BUT)of the modified parks group were significantly higher at 7 days after operation.The score of corneal fluorescein staining of cornea(FL)was significantly lower than that of the control group(P<0.05);the spherical diopter and cylinder diopter of the modified Parks group were significantly lower than those of the control group one month after operation(P<0.05);the total incidence of complications,such as conjunctival swelling,infection,dry eye,and discomfort in the modified Parks group was significantly lower than that in the control group(P<0.05).Conclusion:The modified Parks incision used in the surgical treatment of strabismus in children can reduce the impact on the children’s diopter,maintain the stability of the tear film,and reduce the incidence of complications,which has high application value.Objective:To analyze the effect of modified Parks incision on tear film stability and diopter in children with strabismus.
