摘要
目的分析早孕期系统超声筛查诊断胎儿中枢神经系统畸形的临床价值。方法选取2018年4月至2020年6月本院应用规范化早孕期胎儿结构筛查的孕妇1600例(单胎1200例,双胎400例),共2000胎作为研究对象,按照随机数字表法分为实验组与对照组,每组1000名。对照组常规筛查4个标准切面,实验组在对照组基础上重点观察孕早期超声软标记,比较两组胎儿中枢神经系统畸形检出率及诊断价值。结果实验组检出率为6.00%,明显高于对照组的3.30%,差异有统计学意义(P<0.05)。实验组诊断灵敏度为85.71%,特异度及阳性预测值均为100.00%,阴性预测值为93.00%;对照组诊断灵敏度为86.54%,特异度及阳性预测值均为100.00%,阴性预测值为99.48%;两组诊断灵敏度、特异度及阳性预测值和阴性预测值比较差异无统计学意义。结论早孕期系统超声筛查对胎儿中枢神经系统畸形效果较好,但也存在一定的局限性,需配合中晚孕期超声筛查结果进行精确筛查,进而提高检查精确度。
Objective To analyze the clinical value of systematic ultrasound screening in the diagnosis of fetal central nervous system malformations in early pregnancy.Methods From April 2018 to June 2020,1600 pregnant women(1200 singletons and 400 twins)who applied standardized fetal structure screening during early pregnancy were selected as research subjects,a total of 2000 fetuses and they were divided into experimental group and control group according to the random number table method,with 1000 cases in each group.The control group was routinely screened with four standard sections,and the experimental group focused on observing ultrasound soft labeling in the first trimester on the basis of the control group.The detection rate and diagnostic value of fetal central nervous system malformations were compared between the two groups.Results The detection rate of the experimental group was 6.00%,which was significantly higher than 3.30%of the control group,and the difference was statistically significant(P<0.05).The diagnostic sensitivity of the experimental group was 85.71%,the specificity and positive predictive values were 100.00%,and the negative predictive value was 93.00%;the diagnostic sensitivity of the control group was 86.54%,the specificity and positive predictive values were 100.00%,and the negative predictive values were 99.48%;there were no significant differences in diagnostic sensitivity,specificity,and positive and negative predictive values between the two groups.Conclusion Systematic ultrasound screening in early pregnancy has a good effect on fetal central nervous system malformations,but it also has certain limitations,and it is necessary to accurately screen with the results of ultrasound screening in middle and late pregnancy,so as to improve the accuracy of examination.
