不同剂量维生素C结合辅酶Q10对小儿病毒性心肌炎外周血象及免疫功能的影响研究 被引量:2

摘要 目的观察不同剂量维生素C结合辅酶Q10治疗小儿病毒性心肌炎的效果及对患儿外周血象、免疫功能的影响。方法本次研究选择2021年6月至2022年6月南阳医学高等专科学校第一附属医院收治的126例病毒性心肌炎患儿作为研究对象,通过计算机随机分组法将入组患儿分为对照组(63例,采用常规剂量维生素C结合辅酶Q10治疗)和观察组(63例,采用大剂量维生素C结合辅酶Q10治疗),比较两组患儿治疗前后的外周血象、免疫功能、心肌酶谱及用药安全性。结果观察组治疗1周后微小RNA(MicroRNA-133,miR-133)(3.35±0.27)高于对照组(2.46±0.33),miR-155水平(2.23±0.44)低于对照组(3.41±1.06),治疗2周后的miR-133(4.02±1.25)高于对照组(3.47±0.61),miR-155水平(0.92±0.11)低于对照组(1.58±0.63),差异具有统计学意义(P<0.05)。观察组治疗后的T淋巴细胞表面抗原_(3)+(CD_(3)+)(65.44±10.46)%高于对照组(58.44±10.33)%,T淋巴细胞表面抗原4+(CD_(4)+)(37.25±10.31)%高于对照组(30.22±10.33)%,T淋巴细胞表面抗原8+(CD_(8)+)(42.33±10.16)%低于对照组(49.15±10.47)%,差异具有统计学意义(P<0.05)。观察组治疗后的肌酸激酶(CK)(42.33±5.62)U/L低于对照组(45.75±5.32)U/L,肌酸激酶同工酶(CK-MB)(32.35±5.27)U/L低于对照组(35.46±5.37)U/L,乳酸脱氢酶(LDH)(83.46±10.27)U/L低于对照组(90.41±10.33)U/L,差异具有统计学意义(P<0.05)。治疗后,观察组的药物相关副反应发生率7.94%(5/63)略高于对照组6.35%(4/63),差异无统计学意义(P>0.05)。结论大剂量维生素C结合辅酶Q10可显著改善病毒性心肌炎患儿的外周血象、免疫功能及心肌酶谱指标,可在不增加药物副反应发生风险同时增加临床获益。 Objective To observe the effect of different doses of vitamin C combined with coenzyme Q10 on the treatment of viral myocarditis in children and its impact on peripheral hemogram and immune function.Methods In this study,126 children with viral myocarditis admitted to the First Affiliated Hospital of Nanyang Medical College from June 2021 to June 2022 were selected as the research subjects.The enrolled children were randomLy divided into a control group(63 cases,treated with conventional doses of vitamin C combined with coenzyme Q10)and an observation group(63 cases,treated with high-dose vitamin C combined with coenzyme Q10)by computer randomization.The peripheral hemogram,immune function,myocardial enzyme spectrum,and medication safety of the two groups of children before and after treatment were compared.Results After one week of treatment,the microRNA 133(miR-133)(3.35±0.27)in the observation group was higher than that in the control group(2.46±0.33),and the miR-155 level(2.23±0.44)was lower than that in the control group(3.41±1.06).After two weeks of treatment,the miR-133(4.02±1.25)was higher than that in the control group(3.47±0.61),and the miR-155 level(0.92±0.11)was lower than that in the control group(1.58±0.63),with statistically significant differences(P<0.05).After treatment,the T lymphocyte surface antigen 3+(CD_(3)+)(65.44±0.46)%in the observation group was higher than that in the control group(58.44±10.33)%,the T lymphocyte surface antigen 4+(CD_(4)+)(37.25±10.31)%was higher than that in the control group(30.22±10.33)%,and the T lymphocyte surface antigen 8+(CD_(8)+)(42.33±10.16)%was lower than that in the control group(49.15±10.47)%,with statistically significant differences(P<0.05).After treatment,the creatine kinase(CK)(42.33±5.62)U/L in the observation group was lower than that in the control group(45.75±5.32)U/L,creatine kinase isoenzyme(CK-MB)(32.35±5.27)U/L was lower than that in the control group(35.46±5.37)U/L,and lactate dehydrogenase(LDH)(83.46±10.27)U/L was lower than that in the control group(90.41±10.33)U/L,with statistically significant differences(P<0.05).Conclusion High doses of vitamin C combined with coenzyme Q10 can significantly improve peripheral hemogram,immune function,and myocardial zymogram indicators in children with viral myocarditis,which can increase clinical benefits without increasing the risk of drug side effects.
