摘要
目的:探讨双参数磁共振成像(biparametric magnetic resonance imaging,Bp-MRI)技术对前列腺癌(prostate cancer,PCa)的诊断价值。方法:回顾性收集2019年1月-2021年12月广东省中医院二沙岛医院收治的182例前列腺疾病患者,两名医师采用双参数磁共振成像(Bp-MRI)技术、多参数磁共振成像(multiparameter magnetic resonance imaging,Mp-MRI)技术、T_(2)加权联合动态增强磁共振成像技术在未知病理情况下分别对患者进行前列腺影像报告和数据系统(PI-RADS)评分,以病理结果为金标准,对比分析各影像技术对PCa的诊断效能。结果:两名医师使用Bp-MRI、Mp-MRI、T_(2)WI-DCE技术诊断PCa的AUC值分别为0.88、0.90、0.84,其中,Bp-MRI诊断PCa灵敏度、特异度为0.83、0.86,Mp-MRI为0.85、0.85,T_(2)WI-DCE为0.73、0.88,Bp-MRI与T_(2)WI-DCE的AUC差异具有统计学意义(P<0.05),Bp-MRI与Mp-MRI的AUC差异无统计学意义(P>0.05)。结论:Bp-MRI技术对PCa具有良好的诊断价值,在某些条件下,将Bp-MRI技术作为Mp-MRI技术的备选方案是可行的。
Objective To investigate the diagnostic value of biparametric magnetic resonance imaging(Bp-MRI)technology in prostate cancer(PCa).Methods A total of 182 patients with prostate diseases admitted to Ersha Island Branch of Guangdong Provincial Hospital of Chinese Medicine from January 2019 to December 2021 were collected.Two radiologists used biparametric magnetic resonance imaging(Bp-MRI)technology,MultiParameter magnetic resonance imaging(Mp-MRI)technology and T_(2)-weighted combined dynamic contrast-enhanced magnetic resonance imaging technology to evaluate the prostate imaging reporting and data system(PI-RADS)scores of the above patients without pathological results.The pathological results were used as the gold standard to compare the diagnostic efficacy of each imaging technology for PCa.Results The AUC values of Bp-MRI,Mp-MRI and T_(2)WI-DCE in the diagnosis of PCa by two radiologists were 0.88,0.90 and 0.84,respectively.The sensitivity and specificity of Bp-MRI in the diagnosis of PCa were 0.83 and 0.86,and those of Mp-MRI were 0.85 and 0.85,respectively,T_(2)WI-DCE was 0.73 and 0.88.The difference of AUC between Bp-MRI and T_(2)WI-DCE was statistically significant(P<0.05),but the difference of AUC between Bp-MRI and Mp-MRI was not statistically significant(P>0.05).Conclusion Bp-MRI technology exhibits a robust diagnostic value for PCa,suggesting that it may be a feasible to use Bp-MRI as an alternative to Mp-MRI under specific conditions.
