3.0T MR动态增强病灶影像下乳腺癌保乳手术切缘阳性的预测因素 被引量:2

摘要 目的:探讨3.0T MR动态增强病灶影像下乳腺癌保乳手术切缘阳性的预测因素。方法:回顾性分析2020年1月-2023年3月于深圳市宝安区中医院接受保乳手术治疗的75例乳腺癌患者(14例切缘阳性),所有患者均经手术病理检查确诊。所有患者在术前均接受MRI动态增强扫描,经单因素分析不同MRI影像学征象患者的保乳术切缘阳性影响因素,包括肿瘤强化类型、肿瘤周围点状强化情况、肿瘤大小、动态增强曲线、表观弥散系数值,通过多因素Logistic回归分析影响乳腺癌保乳术切缘阳性的MRI征象。结果:单因素分析结果显示,强化类型非肿块型、肿瘤周围点状强化乳腺癌患者的保乳术切缘阳性率较高(P<0.05);而患者不同肿瘤大小、动态增强曲线形态、表观弥散系数值在切缘阳性率方面差异无统计学意义(P>0.05)。多因素Logistic回归分析结果显示,强化类型非肿块型、肿瘤周围点状强化均为影响乳腺癌保乳术切缘阳性的MRI征象(P<0.05)。结论:非肿块型乳腺癌、肿块周围有点状强化是乳腺癌保乳术中切缘阳性的预测因素。
