摘要
目的分析远端缺血适应训练结合综合康复护理对脑卒中足下垂患者的作用。方法将天津北大医疗海洋石油医院神经内科2022年1月至2022年10月期间收治的80例脑卒中足下垂患者,纳入研究并随机分组,对照组40例实施常规方式护理,研究组40例在常规护理基础上实施远端缺血适应训练结合综合康复护理。结果研究组情绪干预、体征监测、吸痰护理、饮食干预、运动干预、远端缺血适应训练、机体功能康复、心理康复等方面护理措施执行率均显著优于对照组(P<0.05),差异有统计学意义。两组患者接受干预3个月后,神经功能NIHSS评分均较干预前显著降低(P<0.05),差异有统计学意义;自理能力ADL评分均显著提升(P<0.05),研究组显著优于对照组(P<0.05),差异有统计学意义。两组干预后,生活质量SF-36量表8个维度评分均显著提升(P<0.05),研究组显著高于对照组(P<0.05),差异有统计学意义。两组干预前关节屈曲活动度均受限且接近(P>0.05),差异无统计学意义;干预3个月后均得到显著改善(P<0.05),研究组改善情况显著优于对照组(P<0.05),差异有统计学意义。研究组不良反应发生率仅为7.50%,显著低于对照组的27.50%(P<0.05),差异有统计学意义。结论针对脑卒中足下垂患者予以远端缺血适应训练结合综合康复护理,可以产生显著效果,有助于促进患者早期康复,可以有效改善患者关节屈曲活动度、提升生活质量。该方法具有极大的推广应用价值。
Objective The paper explored effect of remote ischemic preconditioning(RIPC)combined with comprehensive nursing on rehabilitation of patients with acute drop foot.Methods 80 patients with acute drop foot from January 2022 to October 2022 were divided into control group(40 cases receiving routine nursing)and study group(40 cases receiving RIPC combined with comprehensive rehabilitation nursing).Results Nursing execution rate including emotional intervention,physical monitoring,sputum suction care,dietary intervention,exercise intervention,distal ischemic adaptation training,body function rehabilitation,psychological rehabilitation in research group was significantly higher than control group(P<0.05),the difference was statistically significant.3 months after intervention,NIHSS scores of neurological function were significantly lowered(P<0.05),ADL scores of self-care ability were significantly improved(P<0.05),with study group significantly better than control group during the same period(P<0.05).Eight dimensional scores of the SF-36 quality of life scale were significantly improved in both groups(P<0.05),with study group significantly higher than control group during the same period(P<0.05).Both groups had limited and close joint flexion activity before intervention(P>0.05),the difference was statistically insignificant,both groups had significant improvement three months after intervention(P<0.05),with study group significantly better than control group(P<0.05).Incidence of adverse reactions in study group was 7.50%,which was significantly lower than 27.50%of control group(P<0.05).Conclusion For patients with acute drop foot,RIPC combined with comprehensive nursing can promote early rehabilitation,effectively improve joint flexion,activity and life quality,it is worthy of promotion.
