摘要
目的探究对高龄股骨粗隆间骨折患者施行人工髋关节置换术、股骨近端髓内钉内固定术治疗的效果。方法使用随机数字表法将2019年1月-2020年10月本院收治的66例高龄股骨粗隆间骨折患者分为对照组和观察组,其中对照组33例行股骨近端髓内钉内固定术治疗,观察组33例行人工髋关节置换术治疗。随访半年,对比两组手术相关指标、髋关节功能评分、并发症发生率。结果两组术前住院时间无显著差异,(P>0.05),观察组手术用时、术中出血量较对照组更多,但术后下床活动时间、住院时长较对照组更短,(P<0.05);观察组术后3个月及术后半年髋关节功能评分较对照组更高,(P<0.05);两组并发症发生率对比无显著差异,(P>0.05)。结论对高龄股骨粗隆间骨折患者施行人工髋关节置换术的效果较股骨近端髓内钉内固定术更佳,术后恢复进程更短,但上述手术用时较长,出血量更多。
Objective To explore the effect of artificial hip replacement and intramedullary nail fixation on the treatment of elderly patients with femoral intertrochanteric fractures.Methods A total of 66 elderly patients with intertrochanteric fracture of femur treated in our hospital from January 2019 to October 2020 were divided into control group(33 cases were treated with proximal femoral nail internal fixation)and observation group(33 cases were treated with hip replacement)by the random number table method.The two groups were followed up for half a year,and the operation related indicators,hip function score and complication rate were compared.Results There was no significant difference in the preoperative hospital stay between the two groups(P>0.05).The operation time was longer and the intraoperative blood loss in the observation group was greater than those in the control group,but the postoperative ambulation time and hospital stay were shorter than those in the control group(P<0.05).The hip function score of the observation group was higher than that of the control group at 3 months and half a year after surgery(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion The effect of artificial hip replacement for elderly patients with femoral intertrochanteric fractures is better than that of proximal femoral nail internal fixation,and the postoperative recovery process is shorter.However,the above-mentioned operations take longer and have more bleeding.
