摘要
介绍谭枚秀教授治疗卵巢过度刺激综合征经验。谭枚秀教授结合多年临床实践经验,从中医辨证论治的角度对OHSS的病因病机进行分析,OHSS是以脏腑功能失常为本,病理产物停滞为标,本虚标实相兼为病。认为病发之初多在肾脾胃,继而累及心肺,终致五脏俱损、气滞、血瘀、水停、痰阻等病理变化。谭枚秀教授治疗OHSS,从整体出发,加以辨证分型,认为OHSS临床常见证有脾肾阳虚、气滞血瘀、阴虚阳亢、气阴衰竭四证。治宜补益脾肾、温阳利水,疏肝理气、养血活血,滋肾益阴、清热抑阳,益气养阴、扶阳固脱。分别予以益肾健脾逐水方、逍遥散合桂枝茯苓丸、滋阴益肾调冲方、生脉散为主方加减治疗。病证相参,以中医药治疗为主,西医药为辅,疗效显著。并附验案二则,以飨同道。
This article introduces Professor Tan Meixiu's experience in treating ovarian hyperstimulation syndrome.Combined with many years of clinical practice experience,professor Tan Meixiu analyzed the etiology and pathogenesis of OHSS from the perspective of TCM syndrome differentiation and treatment.OHSS is based on the dysfunction of viscera,the stagnation of pathological products,and the deficiency and the reality of OHSS are both diseases.It is be-lieved that the disease occurs at the beginning of the kidney,spleen and stomach,then involves the heart and lung,and eventually causes the damage of all the five viscera,Qi stagnation,blood stasis,phlegm and other pathological changes.According to the overall condition of patients,professor Tan Meixiu treated OHSS and differentiated the syndromes.She believed that OHSS often witnessed four syndromes including spleen and kidney yang deficiency,Qi stagnation and blood stasis,Yin deficiency and yang hyperactivity,and qi and yin failure.Treat appropriate tonifying spleen and kidney,warm Yang li water;Soothing the liver and regulating Qi,nourishing blood and activating blood;Nourishing kidney Yin,clearing heat and suppressing yang;nourishing Qi and nourishing Yin,helping Yang gu tuo.The main prescriptions of yi Yin yi shen tiao chong prescription and sheng mai powder were added and subtraction respectively.The treatment of disease and syndrome is mainly based on traditional Chinese medicine,supplemented by western medicine,and the curative effect is remarkable.And attached to the case of 2,to cater to fellow.
