摘要
目的基于德尔菲法构建带状疱疹后遗神经痛评估护理方案,并经临床应用检验其效果。方法通过文献检索,筛选带状疱疹后遗神经痛评估护理相关文献,提取护理证据,选取相关领域权威医务人员成立专家组,经两轮专家函询对指标进行评价、修订,进而形成带状疱疹后遗神经痛评估护理方案。按照组间基本特征具有可比性的原则,从2020年1—12月、2021年1—12月收治的带状疱疹后遗神经痛患者中各选50例作为对照组与观察组,对照组采取常规护理,观察组自2021年1月起应用基于德尔菲法构建的带状疱疹后遗神经痛评估护理方案,比较两组患者疼痛改善效果及护理满意程度。结果研究第1、2轮函询问卷回收率均为100%,专家权威度分别为0.899、0.904,函询意见肯德尔和谐系数分别为0.359(χ^(2)=58.493,P<0.001)、0.612(χ^(2)=92.035,P<0.001),经两轮专家函询最终得到一级指标5个,二级指标13个,三级指标37个,指标变异系数为0.060~0.144。应用结果显示,观察组疼痛改善效果优于对照组,差异有统计学意义(P<0.05);观察组护理满意程度优于对照组,差异有统计学意义(P<0.05)。结论研究基于德尔菲法构建的带状疱疹后遗神经痛评估护理方案内容全面,且科学性、可靠性强,其在临床护理中的应用,能缓解患者疼痛,有助于提升护理满意程度。
Objective To construct an evaluation nursing plan for postherpetic neuralgia based on Delphi method method,and test its effect through clinical application.Methods Through document retrieval,we screened the literature related to the evaluation and nursing of postherpetic neuralgia,extracted nursing evidence,selected authoritative medical personnel in related fields to establish an expert group,evaluated and revised the indicators through two rounds of expert correspondence,and then formed the evaluation and nursing plan for postherpetic neuralgia.According to the principle of comparability of basic characteristics among groups,50 patients with postherpetic neuralgia were selected from January to December 2020 and 50 patients with neuralgia after shingles were selected from January to December 2021 as the control group and the observation group respectively.The control group took routine nursing care.The observation group applied the Delphi method based nursing program to evaluate postherpetic neuralgia from January 2021 to compare the pain improvement effect and nursing satisfaction of the two groups.Results The recovery rate of the first and second rounds of correspondence questionnaires was 100%,the expert authority was 0.899 and 0.904 respectively,and the Kendal concordance coefficient of correspondence opinions was 0.359(χ^(2)=58.493,P<0.001)and 0.612(χ^(2)=92.035,P<0.001)respectively.After two rounds of expert correspondence,five first level indicators,13 second level indicators,and 37 third level indicators were finally obtained,and the index variation coefficient was 0.060~0.144.The application results showed that the pain improvement effect of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05);the nursing satisfaction of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The study based on the Delphi method to construct a comprehensive,scientific and reliable nursing plan for the evaluation of postherpetic neuralgia.Its application in clinical nursing can alleviate patients'pain and help to improve nursing satisfaction.
