摘要
目的:分析瘢痕妊娠患者临床使用彩色多普勒超声的诊断效果及价值。方法:选取2018年3月-2023年2月江苏省镇江市第四人民医院收治的疑似瘢痕妊娠患者65例为观察对象,均进行彩色多普勒超声检查,以病理检查结果为金标准,分析彩色多普勒超声的检出效果及诊断价值。结果:65例疑似瘢痕妊娠患者,病理检查确诊为阳性50例(76.92%),阴性15例(23.08%);50例阳性患者中Ⅰ型21例,Ⅱ型16例,Ⅲ型13例。彩色多普勒超声检出阳性48例(73.85%),阴性14例(21.54%)。彩色多普勒超声灵敏度、准确度、特异度、阳性预测值与病理检查比较,差异无统计学意义(P>0.05);彩色多普勒超声对孕囊型、混合型瘢痕妊娠诊断符合率与病理检查比较,差异无统计学意义(P>0.05)。结论:彩色多普勒超声技术诊断瘢痕妊娠患者的灵敏度、准确度、特异度、诊断符合率等均较高,能够体现妊娠囊位置,可为医师制定治疗方案提供依据。
Objective:To analyze the clinical diagnostic effect and value of color Doppler ultrasound in scar pregnancy patients.Methods:Sixty-five patients with suspected scar pregnancy admitted to Zhenjiang Fourth People's Hospital from March 2018 to February 2023 were selected as the observation objects,and all of them underwent color Doppler ultrasound.The detection effect and diagnostic value of color Doppler ultrasound were analyzed with the results of pathological examination as the gold standard.Results:Among 65 patients with suspected scar pregnancy,50 cases(76.92%)were positive and 15 cases(23.08%)were negative.Among the 50 positive patients,21 cases were typeⅠ,16 cases were typeⅡ,and 13 cases were typeⅢ.Color Doppler ultrasound was positive in 48 cases(73.85%)and negative in 14 cases(21.54%).There was no significant difference in sensitivity,accuracy,specificity and positive predictive value between color Doppler ultrasound and pathological examination(P>0.05).There was no significant difference between color Doppler ultrasound and pathological examination in the diagnosis of pregnancy with cystic and mixed scar(P>0.05).Conclusion:The sensitivity,accuracy,specificity and diagnosis coincidence rate of color Doppler ultrasound in the diagnosis of scar pregnancy patients are high,which can reflect the location of pregnancy sac and provide a basis for doctors to make treatment plans.
