摘要
目的探索完全植入式静脉输液港(totally implantable venous-access port,TIVAP)的技术学习曲线,对比经外周静脉穿刺的中心静脉置管(peripherally inserted central venous catheter,PICC)在乳腺癌新辅助化疗中的应用效果。方法选取2019年7月至2020年12月在赣州市人民医院甲状腺乳腺外科住院的217例乳腺癌新辅助化疗患者,根据中心静脉置管方式分为PICC组(n=75)和TIVAP组(n=142)。比较两组置管成功率和术中、术后并发症,以及患者带管生活质量。结果两组的置管成功率、术中并发症比较,差异均无统计学意义(P>0.05)。TIVAP组术后并发症高于PICC组,留置时间长于PICC组,生活质量优于PICC组,差异均有统计学意义(P<0.05)。结论TIVAP经过早期探索达技术稳定后,置管耗时、成功率均与PICC相近,具有留置时间长、并发症少、满意度高等优点,值得临床普及应用。
Objective To explore the technical learning curve of the totally implantable venous access port(TIVAP),and to compare the application effect of peripherally inserted central catheter(PICC)in neoadjuvant chemotherapy of breast cancer.Methods A total of 217 patients with neoadjuvant chemotherapy for breast cancer who were hospitalized in the department of thyroid and breast surgery of Ganzhou People’s Hospital from July 2019 to December 2020 were selected and divided into PICC group(n=75)and TIVAP group(n=142)according to the type of central venous catheter.The success rate of catheter placement,intraoperative and postoperative complications,and quality of life of patients with catheters were compared between the two groups.Results Compared with the two groups,there was no statistically significant difference in the success rate of catheter placement and intraoperative complications(P>0.05).The TIVAP group had higher postoperative complications than the PICC group,stayed in place longer than the PICC group and had better quality of life than the PICC group,all with statistically significant differences(P<0.05).Conclusion After early exploration and technical stability,there is not difference in catheterization time and success rate compared between TIVAP and PICC.TIVAP had the advantages of longer indwelling time,fewer complications and higher satisfaction.It’s worthwhile to popularise its use in the clinic.
