摘要
目的:系统评价清金化痰汤加减辅助治疗脑卒中相关性肺炎(SAP)的有效性和安全性。方法:计算机检索中国知网(CNKI)、万方数据库(Wanfang)、维普中文科技期刊数据库(VIP)、中国生物医学文献数据库(SinoMed)、PubMed、Cochrane library、Web of Science数据库,检索时间均为建库至2020年11月28日。收集所有清金化痰汤加减治疗SAP的随机对照试验(RCT)研究文献,采用RevMan 5.4软件进行Meta分析。结果:共检索到70篇文献,最终纳入15篇文献,共计1320例患者,其中联合组660例,对照组660例。Meta分析结果显示,清金化痰汤加减联合西医常规治疗可提高SAP临床治疗的有效率、降低患者的临床肺部感染评分(CPIS)、美国国立卫生院神经功能缺损评分(NHISS)及血清C反应蛋白(CRP)、降钙素原(PCT)等炎症因子水平;在降低病死率、缩短住院天数、不良反应发生等方面,两组比较无统计学差异(P>0.05)。结论:清金化痰汤加减联合西医常规治疗SAP可提高临床有效率、改善患者临床症状。在临床实践中可通过中医药治疗来提高SAP的临床有效率。
Objective:To systematically evaluate the efficacy and safety of Qingjin Huatan decoction in the treatment of stroke-associated pneumonia(SAP).Methods:Databases,including CNKI,Wanfang,VIP,SinoMed,PubMed,Cochrane library,Web of Science,were systematically retrieved starting from the database establishment to November 28,2020.All randomized controlled trials(RCT)were collected and analysis were conducted by RevMan 5.4 software for Meta-analysis.Results:A total of 70 articles were retrieved,and 15 RCT involving 1320 patients were selected,including 660 cases in combination group and 660 cases in control group.The results of Meta-analysis showed that Qingjin Huatan decoction combined with conventional treatment could improve the clinical efficacy,decrease clinical pulmonary infection score(CPIS),National Institute of Health Stroke Scale(NHISS),C reactive protein(CRP),and procalcitonin(PCT),as well as other inflammatory markers.No significant difference was identified in mortality rate,length of hospitalization and occurrence of adverse reactions(P>0.05).Conclusion:Qingjin Huatan decoction combined with conventional treatment of SAP is better than conventional treatment alone in improving clinical efficacy and clinical symptoms.In clinical practice,traditional Chinese medicine can be used to improve the clinical efficacy of stroke-associated pneumonia.
