摘要
目的研究闭环式血糖管理对糖尿病患者围术期的血糖控制的应用效果。方法选择2020年4月至2021年4月接受手术治疗并伴发糖尿病的140例患者作为研究对象,按随机数表法分为对照组与观察组,每组70例,给予对照组常规围术期护理,在此基础上添加闭环式血糖管理对观察组进行干预,对2组干预前后的血糖水平进行统计,同时比较2组的低血糖发生率、切口愈合不良事件发生率、切口愈合时间及自我管理水平。结果护理后,观察组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于对照组(P<0.05);观察组的低血糖发生率、切口感染、切口愈合延迟均低于对照组(P<0.05);观察组的切口愈合时间短于对照组(P<0.05);观察组护理后糖尿病管理自我效能量表中的用药、饮食、血糖测量、健康行为评分均高于对照组(P<0.05);观察组的护理满意度高于对照组(χ^(2)=5.833,P<0.05)。结论闭环式血糖管理对糖尿病患者围术期的血糖控制及低血糖发生率均具有良好影响,同时还可提高患者的自我管理水平。
Objective To explore the application effect of closed-loop management of perioperative blood glucose control in patients with diabetes mellitus.Methods A total of140 patients with diabetes mellitus who underwent surgical treatment in our hospital from April 2020 to April 2021 were recruited.They were randomly assigned into the control group(n=70)and the observation group(n=70).Conventional perioperative nursing was given to all recruited patients,and closed-loop blood glucose management was additionally given to those of observation group.Blood glucose levels of the two groups before and after the intervention were measured.The incidences of hypoglycemia,and wound healing adverse events,incision healing time and the Diabetes Management Self-Efficacy Scale(DMSES)scores were compared between groups.Results After nursing,the fasting blood glucose,2-hour postprandial blood glucose,and hemoglobin A1C(HbA1C)in the observation group were significantly lower than those in the control group(P<0.05).The incidence of hypoglycemia,wound infection and wound healing delay in the observation group were significantly lower than those in the control group(P<0.05).The wound healing time in the observation group was significantly shorter than that in the control group(P<0.05).The scores of medication,diet,blood glucose measurement and health behavior in the DMSES after nursing in the observation group were all significantly higher than those in the control group(P<0.05).The nursing satisfaction in the observation group was significantly higher than that in the control group(χ^(2)=5.833,P<0.05).Conclusion Closed-loop blood glucose management has a good effect on perioperative blood glucose control in patients with diabetes mellitus.It can not only reduce the occurrence of hypoglycemia,but also enahance the self-management ability.
