uRT-TPOIS和Monaco计划系统在肺癌旋转调强放疗中的剂量学比较

Comparison of uRT-TPOIS and Monaco Treatment Planning System in Lung Cancer Volumetric Modulated Arc Therapy Planning

摘要 目的比较u RT-TPOIS和Monaco两种治疗计划系统在肺癌旋转调强放疗(Volumetric Modulated Arc Therapy,VMAT)计划中的剂量学差异,为临床应用提供参考。方法选取16例在河南省人民医院放疗科接收治疗的肺癌患者,分别使用Monaco 5.11和u RT-TPOIS两种治疗计划系统设计VMAT计划,比较两个计划的靶区和危及器官(Organs at Risk,OARs)受照剂量、适形度指数(Conformity Index,CI)、均匀性指数以及机器跳数。结果Monaco计划较u RT-TPOIS计划的Dmean、Dmin剂量更高,差异有统计学意义(P<0.05)。Monaco计划的V95较u RT-TPOIS计划明显降低,V107明显增加,差异均有统计学意义(P<0.05)。两种计划的V110比较,差异无统计学意义(P>0.05)。u RT-TPOIS计划的CI,肺V10、V5、Dmean,心脏V30、V40、Dmean均优于Monaco计划,差异有统计学意义(P<0.05)。结论两种系统均能满足肺癌VMAT计划需要,但由于治疗计划系统算法、加速器多叶准直器厚度等影响,不同的治疗计划系统存在差异,在OARs保护及其靶区适形度方面,u RT-TPOIS计划优于Monaco计划。 Objective To compare the dosimetric differences between two treatment planning systems(uRT-TPOIS and Monaco)in volumetric modulated arc therapy(VMAT)for lung cancer and to provide a reference for clinical application.Methods Sixteen patients with lung cancer who received treatment in the Department of Radiotherapy in Henan University People’s Hospital were selected to design VMAT plan using Monaco 5.11 and uRT-TPOIS treatment planning systems,respectively,and the target areas and organs at risk(OARs)exposure dose,conformity index(CI),heterogeneity index and the number of machine between the two plans were compared.Results The Dmean and Dmin doses were higher in the Monaco plan compared to the uRT-TPOIS plan,the difference was statistically significant(P<0.05).V95 in Monaco plan was significantly lower than that in uRT-TPOIS plan,and V107 was increased,the difference was statistically significant(P<0.05).There was no statistically significant difference in V110 between the two plans(P>0.05).The CI,lung V10,V5,Dmean,and heart V30,V40,and Dmean of the uRT-TPOIS plan were better than those of the Monaco plan,the difference was statistically significant(P<0.05).Conclusion Both systems can meet the needs of VMAT planning for lung cancer,but due to the influence of treatment planning system algorithms,accelerator multi-lobe collimator thickness,etc.,there are differences between different treatment planning systems.In terms of OARs protection and target conformal degree,uRT-TPOIS plan is superior to Monaco plan.
