摘要
目的 探讨黄芪建中汤联合温针灸治疗脾胃虚弱型耳鸣的效果。方法 选取南安市美林街道社区卫生服务中心2021年1月—2022年9月收治的脾胃虚弱型耳鸣患者60例,依据随机数字表法分成观察组和对照组各30例。对照组给予温针灸治疗,观察组给予黄芪建中汤联合温针灸治疗。比较两组临床疗效以及治疗前后血清5-羟色胺、γ-氨基丁酸和耳鸣障碍量表评分、中医证候积分、抑郁和焦虑自评表评分。结果 观察组疗效优于对照组,差异有统计学意义(P<0.05)。治疗后两组血清5-羟色胺水平均较治疗前下降,γ-氨基丁酸水平均较治疗前上升,且治疗后观察组血清5-羟色胺水平较对照组低,γ-氨基丁酸水平较对照组高,差异均有统计学意义(P<0.05)。治疗后两组耳鸣障碍量表评分和中医证候积分均较治疗前下降,且观察组较对照组低,差异均有统计学意义(P<0.05)。治疗后两组焦虑和抑郁自评量表评分均较治疗前下降,且观察组较对照组低,差异均有统计学意义(P<0.05)。结论 黄芪建中汤联合温针灸治疗脾胃虚弱型耳鸣,较单独使用温针灸的效果更好,且能够减轻患者焦虑和抑郁情绪,值得临床推广应用。
Objective To explore the therapeutic effect of Huangqi Jianzhong Decoction combined with warm acupuncture and moxibustion in the treatment of tinnitus with spleen and stomach weakness type. Methods A total of 60 patients with spleen stomach weakness type tinnitus admitted to Nan'an Meilin Street Community Health Service Center from January 2021 to September 2022 were selected. They were divided into observation group and control group according to random number table method with 30 cases each. The control group was treated with warm acupuncture and moxibustion, and the observation group was treated with Huangqi Jianzhong Decoction combined with warm acupuncture and moxibustion. Compare the clinical efficacy, serum levels of 5-hydroxytryptamine and γ-aminobutyric acid, tinnitus disorder scale scores, Traditional Chinese Medicine syndrome scores, depression and anxiety self-evaluation scale scores before and after treatment. Results The therapeutic effect of the observation group was significantly better than that of the control group(P<0.05). Compare with before treatment, the serum levels of 5-hydroxytryptamine in both groups decreased significantly, and the levels of γ-aminobutyric acid increased significantly after treatment. After treatment, the serum levels of 5-hydroxytryptamine in the observation group were significantly lower than those in the control group, and the level of γ-aminobutyric acid was higher than that of the control group(P<0.05). After treatment, the scores of the tinnitus disorder scale and Traditional Chinese Medicine syndrome scores in both groups decreased significantly compared to before treatment, and the observation group was significantly lower than the control group(P<0.05). After treatment, the scores of the anxiety and depression selfevaluation scale in both groups decreased significantly compared to before treatment, and the observation group was significantly lower than the control group(P<0.05). Conclusion Huangqi Jianzhong Decoction combined with warm acupuncture and moxibustion is more effective than warm acupuncture and moxibustion alone in the treatment of tinnitus with spleen and stomach weakness type, and can reduce patients' anxiety and depression, which is worthy of clinical application.
