摘要
目的:评价药物治疗与射频消融治疗房颤的经济性,为医生和患者选择治疗方案提供经济学参考。方法:将2020年1月至2022年3月心血管内科就诊的房颤患者随机分为药物治疗组(A组)和射频消融治疗组(B组),依据成本-效果分析(CEA)2种方案的短期经济学价值,构建Markov模型,依据成本-效用分析(CUA)2种方案的长期经济学价值。结果:B组对比A组,CEA分析显示增量成本为58065.22元,增量效果为39.33%,增量成本-效果比(ICER)为1476.36元/1个单位有效率;CUA分析显示增量成本为26124.83元,增量效用为2.22个质量调整生命年(QALYs),增量成本-效用比(ICUR)为11767.94元/QALYs。结论:射频消融对比药物治疗房颤的短期经济性取决于患者及其家属对治疗有效率的支付意愿,长期具有经济学优势。
OBJECTIVE To evaluate the economics benefits of drug therapy versus radio frequency ablation for the treatment of atrial fibrillation and to provide an economic reference for physicians and patients to choose treatment options.METHODS Patients with atrial fibrillation attending the cardiovascular medicine department from January 2020 to March 2022 were randomly divided into drug treatment group(Group A)and radiofrequency ablation treatment group(Group B),and the short-term economic value of the 2 options based on cost-effectiveness analysis(CEA)and the long-term economic value of the 2 options based on cost-utility analysis(CUA)were constructed by Markov model.RESULTS In group B compared to group A,CEA analysis showed incremental cost of 58065.22,incremental effectiveness of 39.33%,incremental cost-effectiveness ratio(ICER)of 1476.36/1 unit of efficiency;CUA analysis showed incremental cost of 26124.83,incremental utility of 2.22 quality-adjusted life years(QALYs),incremental cost-utility ratio(ICUR)of 11767.94/QALYs.CONCLUSION The short-term economics of radio frequency ablation versus drug therapy for atrial fibrillation depend on the willingness of patients and their families to pay for the efficiency of the treatment,with a long-term economic advantage.
