摘要
目的 研究新式医疗帮带教学模式在基层医院医师综合能力培养中的教学效果及满意度。方法 选择2020年6月—2023年6月在墨脱县人民医院工作的临床(急诊、内科、外科、妇产科、儿科)医师60例作为研究对象,采用随机数字表法随机分为观察组和对照组各30例。对照组采用传统的以病例为基础的教学(case-based learning,CBL)模式进行综合能力培养,观察组采用新式医疗帮带教学模式进行综合能力培养。1学年培训后,比较两组理论和技能考核成绩、迷你临床演练评估量表(mini-clinical evaluation exercise,Mini-CEX)评估情况,同时分析比较两组对教学模式的主观评价及反馈情况。结果 经过1学年培训,两组理论及技能考核成绩均有显著提高(均P<0.05),且观察组理论及技能考核成绩均显著高于对照组(均P<0.05);观察组Mini-CEX评估量表总评分分值显著高于对照组(P<0.05),且病史询问、体格检查、临床思维、治疗决策、预后指导、人文关怀、整体完成度等7项分值均显著高于对照组(均P<0.05);观察组满意率显著高于对照组(P<0.05)。采用新式医疗帮带教学模式能更好地激发学习兴趣、提高沟通互动和问题解决能力、提高急救技能、增加知识掌握程度、加强临床思维培养、提高科研创新和团队协作能力、提升医学人文素质水平(均P<0.05),并具有与传统教学模式相似的接受度(P> 0.05)。结论 新式医疗帮带教学模式在基层医院医师综合能力培养中教学效果显著、满意度较好,值得推广。
Objective To study the teaching effect and satisfaction of the new medical assistance teaching mode in the training of comprehensive ability of doctors in primary hospitals.Methods 60 clinicians(emergency,internal medicine,surgery,obstetrics and gynecology,and pediatrics) with intermediate titles and below working in Metuo County People's Hospital from June 2020 to June 2023 were selected as research subjects and randomly divided into the experimental group and control group,with 30 in each group.The control group adopted the traditional case-based learning(CBL) mode for comprehensive ability cultivation,while the experimental group adopted the new medical assistance teaching mode.After one year of teaching,theoretical and operational assessment and Mini-CEX scale assessment were carried out.Meanwhile,students' subjective evaluation of the two teaching modes was analyzed.Results After one year teaching,the theoretical and operational scores of the two groups were significantly improved(all P<0.05),the scores of the experimental group were higher than those of the control group(all P<0.05).The total score of Mini-CEX scale in the experimental group was significantly higher than that in the control group(P<0.05),and the seven scores of medical history inquiry,physical examination,clinical thinking,treatment decision,prognosis guidance,humanistic care and overall completion were significantly higher than those in the control group(all P<0.05).The satisfaction rate of the experimental group was significantly higher than that of control group(P<0.05).The adoption of the new medical assistance teaching mode could better stimulate learning interest,improve communication,interaction and problem solving abilities,improve first-aid skills,increase knowledge grasp,strengthen clinical thinking training,improve team cooperation and scientific research innovation ability,improve professional quality and medical humanistic quality(all P>0.05),and had similar acceptance with the traditional teaching mode.Conclusion The new medical assistance teaching mode has remarkable teaching effect and good satisfaction in the training of comprehensive ability of doctors in primary hospitals,which is worth popularizing.
