摘要
目的分析高危型人乳头瘤病毒(HR-HPV)阳性宫颈癌患者免疫细胞与炎症因子特征。方法选取宫颈癌患者196例作为宫颈癌组,宫颈上皮内瘤变(CIN)患者40例作为CIN组,慢性宫颈炎患者40例作为对照组。检测并比较3组研究对象免疫细胞、炎症因子水平。根据组织学类型的不同,将宫颈癌组患者分为鳞癌组和腺癌组,比较鳞癌组和腺癌组患者的免疫细胞、炎症因子水平。对宫颈癌组患者进行HR-HPV检测,根据结果分为阳性组和阴性组,比较两组患者的免疫细胞、炎症因子水平。使用Spearman相关分析HR-HPV感染与宫颈癌患者免疫细胞、炎症因子的相关性。结果宫颈癌组调节性T细胞(Treg)、辅助性T细胞(Th17)、Th17/Treg、Th1、Th2、IL-17、TGF-β、IFN-γ、IL-4水平明显高于CIN组和对照组,Th1/Th2明显低于CIN组和对照组,差异均有统计学意义(P<0.05)。阳性组Treg、Th17、Th17/Treg、Th2、IL-17、TGF-β、IL-4水平明显高于阴性组,Th1/Th2水平明显低于阴性组,差异均有统计学意义(P<0.05)。宫颈癌患者HR-HPV感染与Treg、Th17、Th17/Treg、Th2、IL-17、TGF-β、IL-4水平呈正相关(P<0.05),与Th1/Th2呈负相关(P<0.05)。结论宫颈癌患者存在一定的Th17/Treg、Th1/Th2紊乱,其中HR-HPV阳性可进一步促使患者Th17/Treg、Th1/Th2失衡,提示通过改善HR-HPV阳性宫颈癌患者的免疫微环境,有助于患者的治疗。
Objective To analyze the characteristics of immune cells and inflammatory factors in high-risk human papillomavirus(HR-HPV)positive cervical cancer patients.Methods A total of 196 patients with cervical cancer were selected as cervical cancer group,40 patients with cervical intraepithelial neoplasia(CIN)were selected as CIN group,and 40 patients with chronic cervicitis were selected as control group.The levels of immune cells and inflammatory factors in the three groups were detected and compared.According to different histological types,cervical cancer patients were divided into squamous cell carcinoma group and adenocarcinoma group,and the levels of immune cells and inflammatory factors were compared between squamous cell carcinoma group and adenocarcinoma group.The patients in cervical cancer group were tested HR-HPV,and were divided into positive group and negative group according to the results,the levels of immune cells and inflammatory factors in the two groups were compared.Spearman test was used to analyze the correlations between HR-HPV infection and immune cells,inflammatory factors in patients with cervical cancer.Results Regulatory T cells(Treg),T helper cells(Th)17,Th17/Treg,Th1,Th2,IL-17,TGF-β,IFN-γand IL-4 levels in cervical cancer group were significant higher than those in CIN group and control group,while Th1/Th2 levels were significant lower than those in CIN group and control group,with statistical significance(P<0.05).The levels of Treg,Th17,Th17/Treg,Th2,IL-17,TGF-βand IL-4 in positive group were significantly higher than those in negative group,while the levels of Th1/Th2 were significantly lower than those in negative group,with statistical significance(P<0.05).HR-HPV infection in cervical cancer patients correlated positively with Treg,Th17,Th17/Treg,Th2,IL-17,TGF-βand IL-4 levels(P<0.05),and correlated significantly negatively with Th1/Th2(P<0.05).Conclusion There are certain Th17/Treg and Th1/Th2 disorders in patients with cervical cancer,and HR-HPV positive can further promote Th17/Treg and Th1/Th2 imbalance in patients with cervical cancer,suggesting that improving the immune microenvironment of patients with HR-HPV positive cervical cancer is helpful for the treatment of patients.
