IL-17A基因多态性与儿童鼻咽部肺炎链球菌定植关系的研究 被引量:3

摘要 目的了解我国儿童IL-17A基因多态性与鼻咽部肺炎链球菌定植的关系,为预防肺炎链球菌感染提供新的思路。方法对2019年8月至2021年9月就诊于首都儿科研究所附属儿童医院被呼吸科诊断为“喉软骨软化”“气道异物”及泌尿外科诊断为“隐睾”的5岁以下的115例非呼吸道感染儿童进行鼻咽拭子肺炎链球菌检测,并同时进行IL-17A基因2个单核苷酸多态性位点(-197G>A、-737C>T)进行分析及血清IL-17A水平测定。将患儿分为年龄<3岁和3~<5岁两个年龄段,比较分析不同年龄段患儿不同基因型的鼻咽拭子肺炎链球菌带菌率。结果115份鼻咽拭子标本中,共分离到肺炎链球菌24株,总带菌率为20.9%。115例儿童中共有97例进行了IL-17A基因多态性分析,-197G>A、-737C>T位点中不同基因型之间带菌率比较差异无显著性(P>0.05)。在-197G>A位点,A/A基因型中,3~<5岁患儿血清IL-17A水平低于年龄<3岁患儿,差异有显著性(P<0.05);年龄<3岁、3~<5岁患儿中带菌率比较差异无显著性(P>0.05)。在-737C>T位点,C/T基因型中,3~<5岁患儿血清IL-17A水平低于年龄<3岁患儿,差异有显著性(P<0.05);C/T基因型中,3~<5岁患儿带菌率高于年龄<3岁患儿,差异有显著性(P<0.05)。结论-737C>T位点C/T基因型的<3岁儿童鼻咽部肺炎链球菌带菌率低,可能与血清IL-17A水平高有关。 Objective We analyzed the nasopharyngeal colonization of Streptococcus pneumoniae and its relationship with IL-17A to provide a new idea for preventing Streptococcus pneumoniae infection.Method This study included 115 children under 5 years of age with non-respiratory tract infections who were admitted to our hospital from August 2019 to September 2021.Nasopharyngeal swabs from these children were tested for Streptococcus pneumoniae.In addition,two single nucleotide polymorphisms in the IL-17A gene(-197G>A and-737C>T)were analyzed,and serum IL-17A levels were detected.Result A total of 24 strains of Streptococcus pneumoniae were isolated from 115 nasopharyngeal swabs,and the total bacteria carrying rate was 20.9%.A total of 97 of 115 children were analyzed for IL-17A gene polymorphism.No statistical difference was found in the carrier rate between different genotypes.For-197G>A,children with the A/A genotype aged 3-<5 years had lower serum IL-17A concentrations than children aged<3 years old(P<0.05).For-737C>T,children with the C/T genotype aged 3-<5 years had lower serum IL-17A concentrations than children aged<3 years old(P<0.05),and the carrier rate of Streptococcus pneumoniae in children aged 3-<5 years was higher than children aged<3 years old(P<0.05).Conclusion C/T genotype at-737C>T had a low Streptococcus pneumoniae carrier rate in nasopharynx of children aged<3 years old,which might be related to the high serum IL-17A concentration.However,the exact conclusion that IL-17A is beneficial to the clearance of Streptococcus pneumoniae in nasopharynx needs to be further confirmed by large sample studies.
