摘要
目的分析前列腺特异性膜抗原(PSMA)、增殖细胞核抗原(Ki-67)、α-甲基酰基辅酶A消旋酶(P504s)在前列腺癌组织中的表达及对患者预后的预测价值。方法选取2016年1月至2020年1月,于福建省南平市人民医院进行前列腺癌治疗的患者82例,收集经手术治疗过程中所切取的患者癌组织及癌旁组织。采用免疫组化法测定PSMA、Ki-67、P504s表达,分析其与临床特征之间的关系及预后的预测价值。结果癌组织PSMA、Ki-67、P504s阳性表达率大于癌旁组织PSMA、Ki-67、P504s阳性表达率,差异有统计学意义(P<0.05);前列腺癌组织中PSMA、Ki-67、P504s阳性表达与患者年龄、肿瘤直径之间,差异无统计学意义(P>0.05),前列腺癌组织中PSMA、Ki-67、P504s阳性表达与临床T分期、分化程度、淋巴结转移之间,差异有统计学意义(P<0.05);术后进行3年随访,其年龄、临床T分期、分化程度、淋巴结转移以及高表达PSMA、Ki-67、P504s均与术后3年生存率、病死率相关(P<0.05);受试者工作特征曲线(ROC)曲线分析显示,与PSMA、Ki-67、P504s单项诊断相比,三项联合对前列腺癌的诊断价值较高(P<0.05)。结论PSMA、ki-67、P504s在前列腺癌组织中均呈上调状态,均与患者临床病理特征及预后相关,三者联合检测可为该疾病提供临床有利参考。
Objective To analyze the expression of prostate-specific membrane antigen(PSMA),proliferating cell nuclear antigen 67(Ki-67)andα-methylacyl-coenzyme A racemase(P504s)in prostate cancer tissues and their predictive values for the prognosis of prostate cancer patients.Methods A total of 82 patients who underwent prostate cancer treatment at Fujian Nanping People’s Hospital from January 2016 to January 2020 were selected,and cancerous tissues and paracancerous tissues excised from patients during the surgical treatment were collected.The expression of PSMA,Ki-67 and P504s was measured by immunohistochemistry to analyze its relationship with clinical features and the predictive value of prognosis.Results The positive expression rates of PSMA,Ki-67 and P504s in cancerous tissues were greater than those in paracancerous tissues,with statistically significant differences(P<0.05).There was no statistically significant difference between the positive expression of PSMA,Ki-67 and P504s in prostate cancer tissues and the patients’age and tumor diameter(P>0.05).However,there was statistically significant difference between the positive expression of PSMA,Ki-67 and P504s in prostate cancer tissues and clinical T-stage,differentiation degree and lymph node metastasis(P<0.05).Through a 3-year postoperative follow-up,it was found that patients’age,clinical T-stage,differentiation degree,lymph node metastasis and high expression of PSMA,Ki-67 and P504s were associated with 3-year postoperative survival and mortality(P<0.05).ROC curve analysis revealed that the combination of the three was of higher diagnostic value for prostate cancer compared to PSMA,Ki-67 or P504s alone(P<0.05).Conclusion PSMA,Ki-67 and P504s were all up-regulated in prostate cancer tissues,all of which correlated with the clinicopathological features and prognosis of patients,and the combined detection of the three could provide a favorable reference for clinical purposes in this disease.
