超声引导治疗颈源性头痛研究进展

摘要 颈源性头痛是临床常见疾病,随着电子产品的广泛使用,发病率逐年升高并有年轻化趋势。其发病机制尚不明确,故而临床尚无标准的治疗方案,目前常采用自我锻炼、药物治疗、物理治疗、正骨推拿、手法松解等治疗方法,但部分患者保守治疗后头痛仍得不到有效控制,常需行疼痛介入治疗。超声引导下神经阻滞、关节注射、肉毒素注射、针刀松解、干针、射频治疗、低温等离子治疗是常用疼痛介入方法,具有良好的临床效果。本文回顾近年来超声引导下疼痛介入治疗颈源性头痛的文献,按治疗靶点分类,从星状神经节、颈2背根节/神经根、颈脊神经后支、关节、肌肉及筋膜、其他等方面进行总结,以期为临床工作提供参考。 Cervicogenic headache is an ordinary nosological entity recently identified.Symptoms of cervicogenic headache have become more common with the advent of electronic derivatives.The incidence ratio has increased steadily,and this condition is becoming more prevalent at a younger age.Currently,there is no standard treatment scheme in the clinic because the pathogenesis is unclear.In the contemporary medical paradigm,selfexercise,drug therapy,physical therapy,massage,and manual relaxation are often employed as treatment methods.The headache of some patients,however,cannot be effectively controlled by conservative treatment,and pain intervention therapy is often necessary.Several typical pain interventions have good clinical outcomes,such as ultrasound-guided nerve blocks,joint injections,toxin injections,needle knife lysis,dry needles,radio frequency and coblation therapy.The purpose of this article is to review recent literature related to the use of ultrasound-guided cervical headache intervention and provides a summary of the stellate ganglion,the cervical 2 dorsal root ganglion/nerve root,the cervical spinal nerve posterior branch,zygapophysial joints/atlantoaxial joint,muscle and fascia of the cervical spine.And it classifies according to the therapeutic targets in order to provide a reference for clinical work.
