摘要
目的探讨平衡仪反馈训练联合镜像疗法对脑卒中偏瘫患者的疗效及对肢体运动功能、肌肉协调性的影响。方法选取2022-01—11首都医科大学附属北京康复医院收治的脑卒中偏瘫患者102例,观察组和对照组各51例,对照组患者采用平衡仪反馈训练,观察组患者在对照组的基础上联合镜像疗法,从入院当日开始,治疗为期4周。比较入院当日及治疗4周后治疗有效率、功能性步行量表(FAC)评分、Tinetti步态评估量表(TGA)评分、Fugl-Meyer运动功能量表(FMA)评分,股四头肌、胫骨前肌、腓肠肌肌电积分值(iEMG),脑源性神经营养因子(BDNF)、神经生长因子(NGF)水平。结果治疗4周后观察组有效率(92.16%比76.47%)、FAC评分[(3.72±0.34)分比(2.72±0.61)分]、TGA评分[(9.87±1.14)分比(6.61±1.82)分]、FMA评分[(22.01±3.64)分比(18.94±2.43)分],股四头肌[(0.15±0.72)μV比(0.11±0.15)μV]、胫骨前肌[(0.14±0.34)μV比(0.10±0.17)μV]、腓肠肌[(0.16±0.41)μV比(0.12±0.15)μV]iEMG水平,BDNF[(21.46±4.34)ng/L比(13.06±2.97)ng/L]、NGF[(28.01±4.47)ng/L比(22.05±8.31)ng/L]水平均高于对照组(P<0.05)。结论平衡仪反馈训练联合镜像疗法能有效改善脑卒中偏瘫患者肢体运动功能及肌肉协调性,改善神经功能,提高治疗有效率。
Objective To investigate the effect of balance instrument feedback training combined with mirror therapy on stroke hemiplegia patients and its influence on limb motor function and muscle coordination.Methods Totally 102 patients with hemiplegia after stroke admitted to the Beijing Rehabilitation Hospital Affiliated to Capital Medical University from January 2022 to November 2022 were divided into observation group(n=51)and control group(n=51).The control group was trained by feedback from the balance instrument.The observation group was combined by mirror therapy on the basis of the control group.The intervention lasted for 4 weeks from the day of admission.The effective rate of treatment,the score of functional ambulation category(FAC),the score of Tinetti gait assessment(TGA),the score of Fugl-Meyer assessment(FMA),the integrated electromyographic(iEMG)values of quadriceps femoris,anterior tibial muscle,gastrocnemius muscle,the levels of brain-derived neurotrophic factor(BDNF)and nerve growth factor(NGF)were compared on the day of admission and after 4 weeks of treatment.Results After 4 weeks of treatment,the effective rate(92.16%vs 76.47%),FAC score((3.72±0.34)scores vs(2.72±0.61)scores),TGA score((9.87±1.14)scores vs(6.61±1.82)scores),FMA score((22.01±3.64)scores vs(18.94±2.43)scores),iEMG level of quadriceps femoris((0.15±0.72)μV vs(0.11±0.15)μV),anterior tibial muscle((0.14±0.34)μV vs(0.10±0.17)μV)and gastrocnemius muscle((0.16±0.41)μV vs(0.12±0.15)μV),BDNF((21.46±4.34)ng/L vs(13.06±2.97)ng/L)and NGF((28.01±4.47)ng/L vs(22.05±8.31)ng/L)level in the observation group were higher than those in the control group(P<0.05).Conclusion The balance instrument feedback training combined with mirror image therapy can effectively improve the limb motor function and muscle coordination,improve the nerve function and the treatment efficiency of stroke hemiplegia patients.
