摘要
目的:探讨振幅整合脑电图(aEEG)联合血清神经元特异性烯醇化酶(NSE)、超敏C反应蛋白(hs-CRP)水平对早产儿脑损伤的预测价值。方法:选取2021年4月至2022年12月诊治的63例有脑损伤的早产儿作为观察组,选取同期119例无脑损伤的早产儿作为对照组。于早产儿出生后3 d内进行aEEG和血清NSE、hs-CRP水平测定。以脑损伤为因变量(1=无,2=有),纳入血清NSE、hs-CRP水平和aEEG总分为解释变量,进行Logistic回归分析,基于回归结果,构建三项联合评估值。三项联合评估值=aEEG总分-0.112×NSE水平-0.443×hs-CRP水平。采用ROC曲线分析血清NSE、hs-CRP水平,aEEG总分及三项联合对早产儿脑损伤的预测价值。结果:观察组aEEG总分低于对照组,血清NSE、hs-CRP水平高于对照组(P<0.05)。ROC曲线分析结果显示,血清NSE水平、血清hs-CRP水平、aEEG总分及三项联合预测早产儿脑损伤的AUC(95%CI)分别为0.701(0.623~0.780)、0.706(0.618~0.793)、0.709(0.631~0.787)和0.825(0.760~0.890);与血清NSE、hs-CRP水平和aEEG总分比较,三项联合的AUC较大(P均<0.05)。结论:aEEG总分和血清NSE、hs-CRP水平三项联合有利于早期发现早产儿脑损伤。
Aim:To explore the predictive value of amplitude integrated electroencephalography(aEEG)combined with serum neuron-specific enolase(NSE)and hypersensitive C-reactive protein(hs-CRP)levels in preterm infants′brain injury.Methods:A total of 63 preterm infants diagnosed as brain injury from April 2021 to December 2022 were selected as the observation group,and 119 preterm infants without brain injury in the same period were selected as the control group.aEEG,serum NSE and hs-CRP levels were determined within 3 days after birth.Logistic regression analysis was performed with brain injury as the dependent variable(1=non-injury,2=injury),and serum NSE,hs-CRP levels and aEEG score as the explanatory variables.Based on the Logistic regression model,three-combination assessment=aEEG score-0.112×NSE-0.443×hs-CRP.ROC curves were used to analyze the predictive value of serum NSE,hs-CRP levels,aEEG score and the three-combination risk for brain injury in preterm infants.Results:Compared with those of control group,aEEG score of observation group was lower,while serum NSE and hs-CRP levels were higher(P<0.05).ROC curve analysis results showed that AUC(95%CI)of serum NSE,hs-CRP levels,aEEG score,and three-combination risk were 0.701(0.623-0.780),0.706(0.618-0.793),0.709(0.631-0.787)and 0.825(0.760-0.890),respectively;compared with those of serum NSE,hs-CRP levels and aEEG score,AUC of three-combination risk was higher(P<0.05).Conclusion:The combination of aEEG score,and serum NSE,hs-CRP levels is beneficial to early detection of brain injury in preterm infants.
