摘要
目的探讨二甲双胍联合达格列净治疗对老年2型糖尿病(T2DM)患者血清同型半胱氨酸(Hcy)、超敏C反应蛋白(hs-CRP)、胱抑素C(CysC)水平及糖脂代谢的影响。方法选取2020年1月至2021年1月本院收治的120例老年T2DM患者作为研究对象,随机分为研究组与对照组,每组60例。研究组采用二甲双胍联合达格列净治疗,对照组单独采用二甲双胍治疗,比较两组临床疗效、不良反应、血糖[空腹血糖(FBG)、糖化血红蛋白(HbA1c)、餐后2 h血糖(2 h PG)]水平、血脂指标[三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)]及血清Hcy、CysC、hs-CRP水平。结果治疗后,研究组CysC、hs-CRP、Hcy水平均低于对照组,差异有统计学意义(P<0.05);治疗后,研究组FBG、HbA1c、2 h PG均低于对照组,差异有统计学意义(P<0.05);治疗后,研究组TG、TC、LDL-C水平均低于对照组,HDL-C水平高于对照组,差异有统计学意义(P<0.05);研究组治疗总有效率(91.67%)高于对照组(75.00%),差异有统计学意义(P<0.05);两组不良反应发生率比较差异无统计学意义。结论二甲双胍联合达格列净治疗老年T2DM疗效显著,可有效改善患者糖脂代谢异常,降低血清CysC、hs-CRP、Hcy水平,且安全性高,值得临床推广应用。
Objective To explore the effect of metformin combined with dapagliflozin on serum homocysteine(Hcy),hypersensitive C-reactive protein(hs-CRP),Cystatin C(CysC)and glycolipid metabolism in elderly patients with type 2 diabetes mellitus(T2DM).Methods 120 elderly patients with T2DM treated in our hospital from January 2020 to January 2021 were selected as the research subjects,and they were randomly divided into the study group and the control group,with 60 cases in each group.The study group was treated with metformin combined with dapagliflozin,and the control group was treated with metformin,the clinical efficacy,adverse reactions,blood glucose indexs fasting blood glucose(FBG),glycated hemoglobin(HbA1c),2 h postprandial blood glucose[2 h PG]levels,blood lipid indexs(triglyceride[TG],total cholesterol[TC],low density lipoprotein cholesterol[LDL-C],high density lipoprotein cholesterol[HDL-C])and serum Hcy,CysC,hs-CRP levels were compared between two groups.Results After treatment,the levels of CysC,hs-CRP and Hcy in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of FBG,HbA1c,2 h PG in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of TG,TC and LDL-C in the study group were lower than those in the control group,the level of HDL-C was higher than that in the control group,and the differences were statistically significant(P<0.05).The total effective rate of the study group(91.67%)was higher than that of the control group(75.00%),and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Metformin combined with dagliazine is effective in the treatment of elderly T2DM,which can effectively improve abnormal glucose and lipid metabolism and reduce serum CysC,hs-CRP and Hcy levels,and is safe and worthy of clinical application.
