摘要
辨病识机是中医认识疾病的重要部分,阴阳学说是中医辨识疾病的本体结构理论。从阴阳辨治肿瘤有利于发掘肿瘤的病机本质,阴阳动态变化贯穿肿瘤疾病发生发展的全过程。笔者根据文献及临床实践凝练出阳虚阴结是结直肠癌的核心病机,其中阳虚是其发生的根源,阴邪凝聚形成的“阴瘤”是结直肠癌局部病灶的本质属性,而疾病的寒热表现则是机体内部阴阳盈虚变化的表现,因此临证时寒热辨证同样重要。阳虚阴结是内在病机,因而治疗的总则在于温通,温阳化气、通下泄阴,使阳气恢复通畅,使阴浊之邪去而不留,从而控制结直肠癌的进展。
Distinguishing disease and identifying mechanism is an important part of traditional Chinese medicine(TCM)diagnosis.The theory of yin and yang is the ontology structure theory of TCM.Differentiating and treating tumor from yin and yang is beneficial to explore the nature of tumor pathogenesis and the dynamic changes of yin and yang run through the whole process of the occurrence and development of tumor diseases.According to the literature review and clinical experience,we concluded that deficiency of yang and the stagnation of yin are the core pathogenesis of colorectal cancer,among which yang deficiency is the root cause of its occurrence,'yin tumor"is formed by the condensation of yin and evil,it is the essential attribute of local lesions of colorectal cancer.The external performance of cold and heat of the disease are the manifestations of the changes of yin and yang,so the differentiation of cold and heat syndrome is equally important when we differentiate the syndrome.Deficiency of yang and the stagnation of yin is the internal pathogenesis,so the general principle of treatment is warm,warming yang qi and clearing yin turbidities,so that yang qi can be restored and yin tarbidities can be cleared,and clearing yin turbidities away,so as to control the progression of colorectal cancer.
