摘要
目的 探讨p38MAPK在子痫前期患者外周血及胎盘组织中的表达及对肿瘤坏死因子-α(TNF-α)、干扰素-γ(IFN-γ)的影响。方法 选取淮安市妇幼保健院住院分娩的子痫前期产妇30例为观察1组,重度子痫前期30例为观察2组,随机抽取同期正常行剖宫产分娩的产妇30例为对照组。均测血清p38 MAPK、p-p38 MAPK、TNF-α、IFN-γ水平及胎盘组织p38 MAPK、p-p38 MAPK表达。分析p38MAPK、p-p38 MAPK与TNF-α、IFN-γ的相关性,二元Logistic回归分析重症子痫前期的影响因素。结果 观察1组、观察2组血清p38 MAPK、p-p38 MAPK、TNF-α、IFN-γ水平高于对照组,观察2组高于观察1组(P<0.05);观察1组、观察2组胎盘组织中p38MAPK、p-p38MAPK表达强于对照组,观察2组强于观察1组(P<0.05);血清p38 MAPK、p-p38 MAPK与胎盘组织中p38 MAPK、p-p38 MAPK呈正相关(P<0.05);观察1组、观察2组血清及胎盘组织中p38 MAPK、p-p38 MAPK与血清TNF-α、IFN-γ水平呈正相关(P<0.05);妊娠20周后产检次数、血清p38 MAPK、p-p38 MAPK是重度子痫前期的独立影响因素(P<0.05)。结论 子痫前期患者外周血及胎盘组织中p38 MAPK、p-p38 MAPK均呈高表达,且其表达水平与血清TNF-α、IFN-γ密切相关,以上因素共同参与子痫前期的发生、发展过程。
Objective To investigate the expression of p38 MAPK in peripheral blood and placental tissue of patients with preeclampsia and its effect on tumor necrosis factor-α(TNF-α)and interferon-γ(IFN-γ).Methods 30 women with pree-clampsia who delivered in Huai’an Maternal and Child Health Hospital from January to July 2022 were selected as observation group 1,30 women with severe preeclampsia were selected as observation group 2,and 30 women with normal cesarean sec-tion were randomly selected as control group.Venous blood samples were collected from all puerpera to measure serum levels of p38 MAPK,p-p38 MAPK,TNF-αand IFN-γ.And the expressions of p38 MAPK and p-p38 MAPK in placenta were deter-mined.The correlation between p38 MAPK,p-p38 MAPK and TNF-α,IFN-γwas analyzed.Binary Logistic regression was used to analyze the influencing factors of severe preeclampsia.Results The levels of serum p38 MAPK,p-p38 MAPK,TNF-αand IFN-γin observation group 1 and 2 were higher than those in control group,and those in observation group 2 were higher than those in observation group 1(P<0.05).The expressions of p38 MAPK and p-p38 MAPK in placenta of observation group 1 and observation group 2 were stronger than those of control group,and observation group 2 was stronger than observation group 1(P<0.05).Serum p38 MAPK and p-p38 MAPK were positively correlated with placental p38 MAPK and p-p38 MAPK(P<0.05).The levels of p38 MAPK and p-p38 MAPK in serum and placental tissue of observation group 1 and observation group 2 were positively correlated with serum levels of TNF-αand IFN-γ(P<0.05).The number of prenatal examinations after 20 weeks gestation,serum p38 MAPK and p-p38 MAPK were independent influencing factors of severe preeclampsia(P<0.05).Conclusion p38 MAPK and p-p38 MAPK expressed highlyin peripheral blood and placental tissue of patients with preeclampsia,and their expression levels are closely related to serum TNF-αand IFN-γ.These factors are involved in the oc-currence and development of preeclampsia.
