摘要
目的探讨三维超声自由解剖成像技术(3DTVUOT)对特殊部位异位妊娠(瘢痕妊娠、宫角妊娠及间质部妊娠)的诊断价值。方法回顾性分析2021年1月至2022年10月南昌仁爱妇产医院收治的经手术病理诊断确诊的33例瘢痕妊娠、43例宫角妊娠及15例间质部妊娠患者的临床资料。所有患者均接受术前二维经阴道超声(2DTVU)及3DTVUOT检查。以手术病理结果为金标准,分析2DTVU及3DTVUOT检查对宫角妊娠、间质部妊娠、瘢痕妊娠的诊断效能。结果33例瘢痕妊娠中孕囊型24例,混合型9例;宫角妊娠孕囊型32例,混合型11例;间质部妊娠孕囊型6例,混合型9例。2DTVU及3DTVUOT检查诊断瘢痕妊娠的准确率为85.71%、93.41%,二者比较差异无统计学意义(P>0.05)。3DTVUOT检查诊断宫角妊娠及间质部妊娠的准确率均为92.31%,高于2DTVU检查的76.92%及78.02%,差异有统计学意义(P<0.05)。结论3DTVUOT检查利于瘢痕妊娠、宫角妊娠、间质部妊娠的早期定性诊断。
Objective To explore the diagnostic value of three-dimensional transvaginal ultrasound omniview technology(3DTVUOT)on ectopic pregnancy at special sites(scar pregnancy,cornual pregnancy and interstitial pregnancy).Methods Clinical data of 33 patients with scar pregnancy,43 patients with cornual pregnancy and 15 patients with interstitial pregnancy treated in the Nanchang Ren'ai Maternity Hospital from January 2021 to October 2022 were retrospectively analyzed.All patients underwent preoperative two-dimensional transvaginal ultrasound(2DTVU)and 3DTVUOT.The diagnostic efficiency of 2DTVU and 3DTVUOT on cornual pregnancy,interstitial pregnancy and scar pregnancy were analyzed by taking surgical-pathological result as the gold standard.Results There were 24 cases of gestational sac type and 9 cases of mixed type among 33 cases of scar pregnancy,and there were 32 cases of gestational sac type and 11cases of mixed type among patients with cornual pregnancy,and there were 6 cases of gestational sac type and 9 cases of mixed type among patients with interstitial pregnancy.The accuracy rates of 2DTVU and 3DTVUOT in the diagnosis of scar pregnancy were 85.71%and 93.41%(P>0.05).The accuracy rates of 3DTVUOT in the diagnosis of cornual pregnancy and interstitial pregnancy were both 92.31%,which were higher than 76.92%and 78.02%in 2DTVU examination(P<0.05).Conclusion 3DTVUOT is beneficial to the early qualitative diagnosis of scar pregnancy,cornual pregnancy and interstitial pregnancy.
