摘要
目的:探讨早期体位活动预防妊高症产妇产后下肢深静脉血栓(Deep venous thrombosis,DVT)的效果及其对凝血功能及下肢股静脉血流速度的影响。方法:选取2020年1月至2022年6月期间我科收治的81例妊高症产妇作为研究对象,按随机数字表法分为按摩组(n=40)和体位组(n=41)。按摩组患者接受产后康复按摩,体位组患者在按摩组的基础上增加早期体位活动干预。对比两组产妇产后下肢肿胀疼痛、凝血功能、下肢股静脉血流速度以及DVT发生情况。结果:干预前,两组下肢肿胀程度、视觉模拟评分(Visual analogue scale,VAS)、凝血酶原时间(Prothrombin time,PT)、活化部分凝血活酶时间(Activated partial thromboplastin time,APTT)、纤维蛋白原(Fibrinogen,FIB)、D-二聚体(D-Dimer,D-D)水平、下肢股静脉血流平均速度、血流峰速度无明显差异(P>0.05);干预后,两组下肢肿胀程度、VAS评分均降低,且体位组低于按摩组(P<0.05);两组PT、APTT均延长,且体位组长于按摩组,两组FIB、D-D水平均降低,且体位组低于按摩组(P<0.05);两组下肢股静脉血流平均速度、血流峰速度均增快,且体位组快于按摩组(P<0.05);体位组DVT发生率明显低于按摩组(P<0.05)。结论:早期体位活动应用于妊高症产妇有助于减轻下肢肿胀和疼痛,改善凝血功能,促进下肢血液循环,降低DVT发生率。
Objective:To investigate the effect of early postural activity for preventing postpartum Deep venous thrombosis(DVT)in patients with pregnant-induced hypertension and its effects on clotting function and blood flow velocity of femoral vein of lower limb.Methods:Atotal of 81 pregnant women with pregnancy-induced hypertension admitted to our department from January 2020 to June 2022 were selected as subjects and divided into massage group(n=40)and postural group(n=41)according to random number table.Massage group patients received postpartum rehabilitation massage,postural group patients on the basis of the massage group to increase early postural activity intervention.Postpartum lower limb swelling and pain,blood clotting function,lower limb femoral vein blood flow velocity and DVT occurrence were compared between the two groups.Results:Before intervention,the degree of lower limb swelling,Visual analogue scale(VAS)score,Prothrombin time(PT),Activated partial thromboplastin time(APTT),Fibrinogen(FIB),D-Dimer(D-D)level,mean velocity and peak velocity of femoral vein blood flow in lower limbs were not significantly different(P>0.05).After intervention,lower limb swelling degree and VAS score were decreased in both groups,and postural group was lower than massage group(P<0.05).PT and APTT in 2 groups were prolonged,and the body position was longer than that in massage group,the FIB and D-D levels in 2 groups were decreased,and the body position group was lower than that in massage group(P<0.05).The average velocity and peak velocity of lower limb femoral vein blood flow were increased in both groups,and the postural group was faster than the massage group(P<0.05).The incidence of DVT in posture group was significantly lower than that in massage group(P<0.05).Conclusion:Early postural activity is helpful for pregnant women with pregnancy-induced hypertension to reduce lower limb swelling and pain,improve clotting function,promote lower limb circulation,and reduce the incidence of DVT.
