摘要
目的:使用数据挖掘的方法,基于证候分布探究针刺治疗多囊卵巢综合征选穴规律。方法:检索中国知网(CNKI)、万方数据平台(WF)、维普全文数据库(VIP)、PubMed与Web Of Science数据库建库至今的针刺辨证治疗PCOS的RCTs,使用Excel、SPSS Modeler 18.0及SPSS Statistics 26.0等软件进行数据分析,探究基于证候分布的针刺治疗多囊卵巢综合征选穴特点及规律。结果:共纳入135篇文献,合计出现1152频次穴位,其中关元穴为使用频次最高的穴位,任脉为涉及最多的经脉,下肢部为取穴涉及最多的部位,最常使用的特定穴为募穴,脾虚痰湿证为涉及最多的证候类型,运用关联规则分析对排名前5的证型进行关联规则分析及聚类分析,以探究不同证型下针刺治疗PCOS的选穴特点及规律。结论:PCOS患者异质性大,不同证型患者的治疗方式亦不相同,针刺辨证治疗PCOS疗效显著,临床需在合理辨证的基础上施针取穴,往往可取得较好的疗效。
Objective:To explore the rules of acupoints selection in treatment of polycystic ovary syndrome(PCOS)based on data mining and syndrome distribution.Methods:RCTs of acupuncture syndrome differentiation in treatment of PCOS were searched from CNKI,Wanfang Data Platform(WF),VIP full-text database,PubMed and Web of Science database since its establishment.Excel,SPSS Modeler 18.0,SPSS Statistics 26.0 and other software were used for data analysis to explore the characteristics and rules of acupoints selection in treatment of PCOS by acupuncture based on syndrome distribution.Results:A total of 135 literature were included,involving 1,152 acupoints,among which Guanyuan(RN4)was the most frequently used acupoint,Ren channel was the most involved meridian,lower extremity was the most involved part of acupoints,the most commonly used specific acupoints were Mu points,Spleen deficiency and phlegm-dampness was the most involved syndrome type,association rule analysis and cluster analysis were conducted on the top 5 syndrome types,in order to explore the acupoints selection characteristics and rules of acupuncture treatment for PCOS of different syndromes.Conclusion:Acupuncture syndrome differentiation has a significant curative effect on PCOS.PCOS patients have a great heterogeneity,and the treatment methods of patients with different syndrome types are also different.Acupoints selection on the basis of reasonable syndrome differentiation can often achieve better curative effects.
