摘要
目的探讨成年男性禁欲天数和年龄对精子活力,特别是前向运动精子总数的交互影响,以指导辅助生殖技术治疗中的最佳禁欲天数。方法选取2020年1月至2022年10月在杭州市妇产科医院生殖医学中心就诊的进行精液常规检查的成年男性8418例,以5周岁为一阶段将8418例成年男性分为5组:≤25周岁、26~30周岁、31~35周岁、36~40周岁和>40周岁;根据WHO建议禁欲2~7 d,将禁欲天数细分为7组:≤2 d、3 d、4 d、5 d、6 d、7 d、>7 d。应用多因素方差分析明确禁欲天数和年龄对成年男性精子活力的交互影响。结果多因素方差分析明确禁欲天数和年龄存在交互作用,对前向运动精子总数的影响有统计学意义(F=1.590,P<0.05,偏η^(2)=0.005)。进一步分析前向运动精子总数的单独效应,不同禁欲天数对≤25周岁(F=3.138,P<0.05,偏η^(2)=0.002)、26~30周岁(F=33.341,P<0.01,偏η^(2)=0.023)、31~35周岁(F=15.342,P<0.01,偏η^(2)=0.011)、36~40周岁(F=13.104,P<0.01,偏η^(2)=0.009)男性前向运动精子总数均有影响;对上述4组中不同禁欲天数的前向运动精子总数进行成对比较,分析得出不同年龄段前向运动精子总数有显著差异的禁欲天数组合。结论禁欲天数和年龄对成年男性前向运动精子总数存在交互作用;在进行辅助生殖技术治疗时,为获得最多数量的前向运动精子,建议≤25周岁男性禁欲7 d,26~40周岁男性禁欲5~7 d,>40周岁男性禁欲3~7 d。
Objective To explore the interactive effects of abstinence days and age on the sperm motility in adult men.Methods A total of 8418 adult men who visited Hangzhou Women's Hospital from January 2020 to October 2022 for routine semen examination were enrolled.The subjects were divided into five age groups:≤25 years,26-30 years,31-35 years,36-40 years,and>40 years.According to abstinence days the subjects were divided into seven groups:≤2 d,3 d,4 d,5 d,6 d,7 d and>7 d.Multivariate ANOVA was used to determine the interaction effect of abstinence days and age on sperm motility in adult men.Results Multivariate analysis showed that there was an interaction effect between abstinence days and age on the total number of forward motility sperm(F=1.590,P<0.05,partialη^(2)=0.005).Further analysis showed that different abstinence days had a significant impact on the total number of forward motility sperm in age groups≤25 years(F=3.138,P<0.05,partialη^(2)=0.002),26-30 years(F=33.341,P<0.01,partialη^(2)=0.023),31-35 years(F=15.342,P<0.01,partialη^(2)=0.011),and 36-40 years(F=13.104,P<0.01,partialη^(2)=0.009).Pairwise comparison analysis showed that to obtain the maximum number of forward moving sperm,abstinence for 7d,5-7 d and 3-7 d might be the optimal length for men aged≤25,26-40,and>40,respectively.Conclusion The number of abstinence days and age have an interaction with the total number of forward motility sperm in adult males.The study recommend the abstinence days in men with different ages to obtain the maximum number of forward moving sperm for assisted reproductive technique.
