摘要
介绍王琛教授运用角药及药对治疗慢性肾功能衰竭的临床经验。王琛教授认为慢性肾功能衰竭的病机关键为虚、湿、瘀、毒,治疗宜标本兼治,扶正固本,祛邪清源。临证有补益脾肾、益气温阳、固精止涩、解表升阳、宣通三焦、祛湿利水、通腑泄浊、活血行气、散结通络、搜风剔络诸法,相应有党参-山药-山茱萸、沉香-肉桂、金樱子-芡实、葛根-升麻-柴胡、杏仁-白豆蔻-薏苡仁、白茅根-藤梨根-薏苡根、大黄-虎杖、桃仁-当归-川芎、大血藤-络石藤-青风藤、九香虫-水蛭诸种角药及药对,临证当证机互参,灵活运用对药及角药组方配伍,起到协同增效的作用。
This paper introduces Professor WANG’s clinical experience in treating chronic renal failure with medicinal pairs and horn medicinals.Professor WANG believes that the pathogenesis of chronic renal failure is deficiency,dampness,stasis and toxin,and in terms of treatment,it is suitable to treat the root and branch simultaneously,reinforce healthy qi and secure the root,and dispel pathogen and clear the source.Clinically,treatment methods are as followed:supplementing and boosting the spleen and kidney,boosting qi and warming yang,consolidating essence and relieving enuresis,releasing the exterior and raising yang,diffusing and unblocking the triple energizers,dispelling dampness and promoting urination,relieving bowels and purging turbidity,invigorating blood and moving qi,dissipating masses and unblocking the collaterals,and searching for and picking out wind pathogen in collaterals.There are correspondent triplet medicinals and couplet medicinal,including Codonopsis Radix-Dioscoreae Rhizoma-Corni Fructus,Aquilariae Resinatum Lignum-Cinnamomi Cortex,Rosae Laevigatae Fructus-Euryales Semen,Puerariae Lobatae Radix-Cimicifugae Rhizoma-Bupleuri Radix,Armeniacae Semen Amarum-Amomi Kravanh Fructus-Coicis Semen,Imperatae Rhizoma-root of Actinidia chinensis Planch.-root of Coicis Semen,Rhei Radix et Rhizoma-Rhizoma Polygoni Cuspidati Semen Persicae-Angelicae Sinensis Radix-Chuanxiong Rhizoma,Sargentodoxae Caulis-Trachelospermi Caulis et Folium-Sinomenii Caulis and Aspongopus-Hirudo.In clinical practice,it is appropriate to take both syndromes and pathogenesis into consideration,and flexibly use couplet medicinal and triplet medicinals,so as to enhance therapeutic effect.
