摘要
目的:探讨社会资本对成都市城市老年人家庭医生签约服务签约情况的影响,为引导老年人接受家庭医生签约服务提供参考。方法:采用多阶段分层整群抽样法抽取调查对象,对成都市60岁及以上的老年人采用面对面问卷调查的方式收集资料,运用单因素分析和非条件logistic回归模型探讨社会资本等因素对家庭医生签约服务签约情况的影响。结果:共得到2148份有效问卷。老年人家庭医生签约服务的签约率为47.3%。年龄(OR=1.231)、慢性病患病情况(患有1-2种慢性病OR=1.871;患有3种及以上慢性病OR=1.677)、吸烟情况(吸烟OR=0.743;已戒烟OR=1.832)以及医疗保险参与情况(OR=1.622)会影响老年人家庭医生签约服务的签约情况;在控制了以上因素后,社会资本中的家庭社会资本(OR=1.031)、社团社会资本(OR=1.107)和社区社会资本(OR=1.134)会对老年人的家庭医生签约服务签约情况产生影响。结论:社会资本对成都市城市老年人家庭医生签约服务签约情况具有正向作用,年龄、慢性病患病情况、吸烟情况及医疗保险参与情况均会影响老年人家庭医生签约服务签约情况,建议通过家庭、社团、社区3个维度的共同联动,构建符合城市老年人特点的家庭医生签约服务促进模式。
Objective:To explore the influence of social capital on the signing status of family doctor contracting services among urban elderly in Chengdu,so as to provide reference for guiding the elderly to contract the family doctors.Methods:The study selected the elderly who were 60 years old and above with multi-stage stratified cluster sampling method and surveyed with face-to-face questionnaire interviews,and explored the influence of social capital and other factors on contracting family doctor behavior with univariate analysis and unconditional logistic regression.Results:A total of 2148 valid questionnaires were obtained.The signing rate of family doctor contracting services among the elderly was 47.3%.The age(OR=1.231),chronic disease(suffered from 1 or 2 chronic diseases OR=1.871,suffered from more than two chronic diseases OR=1.677),smoking(smoking OR=0.743,quit smoking OR=1.832)and social medical insurance purchase(OR=1.622)of the aged residents affected the contract of the family doctor.Community support(OR=1.031),community belonging(OR=1.107)and community cohesion(OR=1.134)in social capital had an impact on aged residents'contracted family doctor services behavior after controlling the above factors.Conclusion:Social capital has a positive effect on the promotion of family doctor contracting services for urban elderly in Chengdu.Age,chronic disease prevalence,smoking status,and medical insurance participation will all affect the signing status of family doctor contracting services.It is suggested to build a model of contracted service promotion for family doctors in line with the characteristics of aged urban residents through the joint linkage of family,community and community dimensions.
