Equity and spatial distribution of allocation of human resources for health in maternal and child health care institutions in China from 2012 to 2020

摘要 [目的]分析我国妇幼保健机构卫生人力资源配置的公平性及空间分布,为优化妇幼人力资源配置提供建议。[方法]采用集聚度指标和空间自相关分析我国妇幼保健机构卫生人力资源配置公平性和空间分布。[结果]我国万人口妇幼卫生技术人员数从2012年的1.74人增长到2020年的3.04人;2020年,全国及东、中、西部地区的医护比分别为1∶1.29、1∶1.20、1∶1.29和1∶1.42,各地区卫生技术人员数的HRAD值分别是:西部地区(0.44)<中部地区(1.94)<东部地区(3.83),HRAD/PAD值:东部地区(0.91)<中部地区(1.00)<西部地区(1.15);全国卫生技术人员Moran's I指数从2012年的0.12增长到2020年的0.27,局部空间自相关分析结果表明:湖南、广西等中西部省份的妇幼卫生人力资源存在空间聚集性。[结论]我国东部地区妇幼卫生人力资源人口配置公平性和医护比有待改善,西部地区地理配置公平性不足,妇幼卫生人力资源配置空间聚集性特征明显。 Objective To analyze equity and spatial distribution of allocation of human resources for health in maternal and child health care institutions in China,so as to provide suggestions for optimizing human resources allocation in maternal and child health care institutions.Methods The agglomeration degree and spatial autocorrelation were used to analyze the equity and spatial distribution of allocation of human resources for health in maternal and child health care institutions in China.Results The number of health technicians for maternal and child health care per 10000 population increased from 1.74 in 2012 to 3.04 in 2020 in China.In 2020,the ratios of doctors to nurses were 1∶1.29,1∶1.20,1∶1.29 and 1∶1.42 in the whole country,the eastern,central and western regions respectively.The HRAD of health technicians in different regions were as follows:western region(0.44)<central region(1.94)<eastern region(3.83);HRAD/PAD:eastern region(0.91)<central region(1.00)<western region(1.15).The Moran s I index of national health technicians increased from 0.12 in 2012 to 0.27 in 2020.The results of the local spatial autocorrelation analysis indicated that there was spatial clustering of human resources for health in maternal and child health care institutions in central and western provinces such as Hunan and Guangxi.Conclusions The equity in the population distribution of human resources for health in maternal and child health care institutions and the ratios of doctors to nurses require improvement in the eastern region.The equity in the geographical distribution is insufficient in the western region.There is a noticeable spatial clustering of allocation of human resources for health in maternal and child health care institutions.
