摘要
目的对比亚洲型股骨近端髓内钉(asian type proximal femoral nail,APFN)与抗旋股骨近端髓内钉(proximal femoral nail antirotation,PFNA)治疗老年股骨转子间骨折的疗效。方法回顾性分析2018年4月—2021年9月连云港市中医院诊治的老年股骨转子间骨折患者96例的临床资料,以不同手术方案分组,对照组56例采用PFNA治疗,治疗组40例采用APFN治疗。对比两组手术相关指标,关节功能评分,疼痛情况及术后并发症指标。结果治疗组术中失血量、透视次数、住院时间均优于对照组,差异有统计学意义(P<0.05)。治疗组术后3、6个月髋关节功能评分量表(Harris Hip Score,HHS)评分高于对照组,差异有统计学意义(P<0.05)。治疗组并发症发生率(7.50%)明显低于对照组(23.21%),差异有统计学意义(χ^(2)=4.149,P<0.05)。结论与PFNA内固定比较,采用APFN治疗老年股骨转子间骨折可有效减少术中失血和透视次数,缩短住院时间,促进髋关节功能恢复,减轻患者术后疼痛,且安全性高。
Objective To compare the efficacy of Asian type proximal femoral nail(APFN)and proximal femoral nail anti-rotation(PFNA)in the treatment of intertrochanteric fracture of femur in the elderly.Methods A retrospective analysis was conducted on the clinical data of 96 elderly patients with intertrochanteric fractures of the femur treated at Lianyungang Traditional Chinese Medicine Hospital from April 2018 to September 2021.They were divided into different surgical plans,with 56 patients in the control group received PFNA treatment and 40 patients in the treatment group received APFN treatment.Compared the two groups of surgical related indicators,joint function scores,pain status,and postoperative complications indicators.Results The intraoperative blood loss,fluoroscopy frequency,and hospital stay in the treatment group were all better than those in the control group,the difference was statistically significant(P<0.05).The Harris Hip Score(HHS)score of the treatment group were higher than those of the control group at 3 and 6 months after surgery,the difference was statistically significant(P<0.05).The incidence of complications in the treatment group(7.50%)was significantly lower than that in the control group(23.21%),the difference was statistically significant(χ^(2)=4.149,P<0.05).Conclusion Compared with PFNA internal fixation,the use of APFN in the treatment of elderly intertrochanteric fractures of femur can effectively reduce intraoperative blood loss and the number of fluoroscopy,shorten the length of hospitalization,promote the recovery of the hip joint function,reduce the patient's postoperative pain,and is safe.
