磁共振波谱与MRI检查在脑肿瘤诊断中的应用

摘要 目的分析磁共振波谱(MRS)与MRI检查在脑肿瘤诊断中的应用价值。方法选取2019年1月至2020年12月吉安市中心人民医院收治的80例脑肿瘤患者作为研究对象,均经手术及病理检查确诊,所有患者均行MRS和MRI检查。分析术后病理检查结果,比较两种不同检查方法对脑肿瘤良、恶性的检出情况及诊断价值,比较良、恶性脑肿瘤患者胆碱(Cho)/肌酸(Cr)比值、N-乙酰天门冬氨酸(NAA)/Cr比值。结果术后病理检查结果显示,80例脑肿瘤患者中,检出恶性肿瘤60例,占比75.00%;检出良性肿瘤20例,占比25.00%。MRS对良、恶性脑肿瘤的检出率分别为90.00%、95.00%,均高于MRI的60.00%、78.33%,差异有统计学意(P<0.05)。MRS对良、恶性脑肿瘤的诊断符合率、灵敏度、特异度和阴性预测值均明显高于MRI,差异有统计学意义(P<0.05),MRS与MRI阳性预测值比较差异无统计学意义。恶性脑肿瘤患者Cho/Cr比值明显高于良性脑肿瘤患者,NAA/Cr比值明显低于良性脑肿瘤患者,差异有统计学意义(P<0.05)。结论MRS检查能提高对脑肿瘤的检出率,减少漏诊或误诊发生,在脑肿瘤诊断中具有较高的应用价值。 Objective To analyze the application value of magnetic resonance spectroscopy(MRS)and MRI in the diagnosis of brain tumors.Methods 80 patients with brain tumors admitted to Ji'an Central People's Hospital from January 2019 to December 2020 were selected as the study subjects,all of whom were confirmed by surgery and pathological examination,MRS and MRI were performed on all patients.The results of postop-erative pathological examination were analyzed,and the detection and diagnostic value of two different examination methods for benign and malig-nant brain tumors were compared,the choline(Cho)/creatine(Cr)ratio and N-acetylaspartate(NAA)/Cr ratio of benign and malignant brain tu-mors were compared.Results Postoperative pathological examination results showed that among the 80 patients with brain tumors,60 cases(75.00%)of malignant tumors were detected,20 cases(25.00%)of benign tumors were detected.The detection rate of MRS for benign and malig-nant brain tumor were 90.00%,95.00%,respectively,which were higher than 60.00%and 78.33%of MRI,and the differences were statistically sig-nificant(P<0.05).The diagnostic coincidence rate,sensitivity,specificity and negative predictive value of MRS for benign and malignant brain tu-mors were significantly higher than those of MRI,the differences were statistically significant(P<0.05),but there was no statistically significant dif-ference in positive predictive value between MRS And MRI.The Cho/Cr ratio of malignant brain tumor patients was significantly higher than that of benign brain tumor patients,and the NAA/Cr ratio was significantly lower than that of benign brain tumor patients,and the differences were statisti-cally significant(P<0.05).Conclusion MRS examination can improve the detection rate of brain tumor,reduce the occurrence of missed diagnosis or misdiagnosis,and has high application value in the diagnosis of brain tumor.
