摘要
目的系统评价A型人格特征与胃癌间的关系,为胃癌的预防提供循证依据。方法检索CNKI、CBM、万方、维普、Web of Science、PubMed等数据库关于A型人格与胃癌相关的文献,检索时限截止为2022年11月。2名研究员背对背筛查研究并对其进行质量评估后,应用Stata 15.0软件开展Meta分析。结果16项病例-对照研究被纳入分析,文献质量均较好。Meta分析结果表明,A型人格会增加胃癌危险(OR=2.01,95%CI:1.67,2.41,P<0.01),亚组分析显示,该结果在南方(OR=2.20,95%CI:1.63,2.99)、北方(OR=1.80,95%CI:1.53,2.11)、平均年龄低于60岁(OR=1.18,95%CI:1.45,2.27)、年龄超过60岁(OR=1.96,95%CI:1.62,2.37)人群中均有统计学意义(P<0.01)。结论A型人格特征与胃癌发病存有明显联系,可能是胃癌的危险因素,该趋势在平均年龄60岁以上及南方地区人群中更显著,但因研究类型及样本量限制,尚需较更高质量的研究去进一步验证。
Objective To systematically evaluate the correlation between type A personality and gastric cancer in China,so as to provide evidence-based basis for the prevention of gastric cancer.Methods The epidemiological studies on the correlation between type A personality and gastric cancer in China published before November 2022 were retrieved from CNKI,CBM,Wanfang,VIP,Web of Science,PubMed and other online databases.After back-to-back screening study and quality assessment by 2 researchers independently,Meta-analysis was carried out with Stata 15.0 software.Results A total of 16 case-control studies were included,and the literature quality was good.Meta-analysis showed that type A personality was a risk factor for gastric cancer in China(OR=2.01,95%CI:1.67,2.41,P<0.01).Subgroup analysis showed that type A personality was significantly correlated with gastric cancer in Chinese population in the Southern region(OR=2.20,95%CI:1.63,2.99,P<0.01),the Northern region(OR=1.80,95%CI:1.53,2.11,P<0.01),and those with the mean age below 60 years(OR=1.18,95%CI:1.45,2.27,P<0.01),and over 60 years(OR=1.96,95%CI:1.62,2.37,P<0.01).Conclusion Type A personality is correlated with the occurrence of gastric cancer,which may be a risk factor for gastric cancer.The correlation is more significant in people over 60 years of age and in Southern China.However,due to the limitations of research type and sample size,higher-quality research is needed for further verification.
