摘要
目的分析磁共振弥散加权成像(MRI-DWI)联合血清前列腺特异性抗原(PSA)诊断前列腺癌的准确率。方法选定无锡市人民医院2020年1月-2023年1月就诊的118例前列腺病变患者,均给予MRI-DWI检查,抽取空腹静脉血5mL,检测血清PSA水平,将手术病理诊断结果作为此次研究的金标准,比较MRI-DWI、PSA、MRI-DWI+PSA诊断前列腺癌准确率、灵敏度、特异度,Kappa检验MRI-DWI、PSA、MRI-DWI+PSA与金标准的一致性。比较良、恶性病变组表观扩散系数(ADC)值、血清PSA水平。结果MRI-DWI+PSA诊断准确率(95.76%)、灵敏度(95.83%)、特异度(95.65%)均高于MRI-DWI(79.66%、83.33%、73.91%)、PSA(68.64%、72.22%、63.04%),P<0.05,Kappa检验MRI-DWI、PSA与金标准的一致性均一般(Kappa值=0.675、0.601),MRI-DWI+PSA与金标准的一致性较好(Kappa值=0.794)。恶性病变组ADC值低于良性病变组,P<0.05,恶性病变组血清PSA水平高于良性病变组,P<0.05。结论MRI-DWI通过定量检测ADC值,联合血清PSA可提高前列腺癌诊断准确率、灵敏度及特异度,弥补了单一检测的不足。
Objective To analyze the accuracy of magnetic resonance diffusion-weighted imaging(MRI-DWI)combined with serum prostate specific antigen(PSA)in the diagnosis of prostate cancer.Methods A total of 118 patients with prostatic lesions admitted to Wuxi People's Hospital from January 2020 to January 2023 were selected to receive MRI-DWI examination,and 5mL fasting venous blood was extracted to detect serum PSA level.The results of surgical pathological diagnosis were used as the gold standard for this study.The accuracy,sensitivity and specificity of MRI-DWI,PSA,MRI-DWI+PSA in the diagnosis of prostate cancer were compared,and the consistency of MRI-DWI,PSA,MRI-DWI+PSA and gold standard was tested by Kappa.The apparent diffusion coefficient(ADC)and serum PSA level were compared between benign and malignant lesions.Results The diagnostic accuracy(95.76%),sensitivity(95.83%)and specificity(95.65%)of MRI-DWI+PSA were higher than those of MRI-DWI(79.66%,83.33%,73.91%)and PSA(68.64%,72.22%,63.04%),P<0.05.In Kappa test,the consistency between MRI-DWI and PSA and gold standard was general(Kappa value=0.675,0.601),while the consistency between MRI-DWI+PSA and gold standard was good(Kappa value=0.794).The ADC value in the malignant lesion group was lower than that in the benign lesion group,P<0.05,and the serum PSA level in the malignant lesion group was higher than that in the benign lesion group,P<0.05.Conclusion MRI-DWI can improve the diagnostic accuracy,sensitivity and specificity of prostate cancer by quantitative detection of ADC value and serum PSA,which makes up for the deficiencies of single detection.
