摘要
近年来,各种现代医学方法使得骨科疾病经过规范化的诊疗得以缓解或痊愈,但仍存在并发症多、治疗手段少等缺陷,中医药治疗骨科疾病则拥有其特色优势。补阳还五汤始见于清代王清任编撰的《医林改错》,是益气活血之代表方,广泛应用于基本病机为气虚血瘀的疾病,该方因其良好的疗效故而在临床多种疾病的治疗中得到广泛应用。同时,该方为心血管科、神经科及内分泌科常用方剂,在一些骨科疾病上缺乏临床数据总结。近几年,该方逐渐应用于骨科临床上,对某些骨科疾病具有较好的疗效和较低的不良反应,因此已经逐渐引起学者及临床医师的关注。文章主要介绍了补阳还五汤在骨科有关病症中的运用,通过剖析其组成及功用、七味中药在骨科有关病症中的作用机制、防治骨科各种病症的现代药理学研究等进行了论述,同时进一步总结了补阳还五汤在某些骨科疾病方面的良好临床疗效,特别是在脊柱疾病、关节炎、肩袖损伤术后、关节置换术后以及术后深静脉血栓形成预防等方面进行逐一分节论述,从中医基础理论依据、现代药理研究证据及临床疗效评估数据等方面展开论述,总结其在骨科优势病种中的重要作用及疗效,以期能进一步开发利用其功效,挖掘其在骨科的应用价值。
In recent years,orthopedic diseases have been alleviated or cured by standardized diagnosis and treatment with various modern medical methods,but there are still many complications,few treatment methods and other defects.Traditional Chinese medicine has its characteristic advantages in the treatment of orthopedic diseases.Buyang Huanwu decoction(补阳还五汤)is first recorded in the Yilin Gaicuo(《医林改错》)compiled by WANG Qingren in the Qing Dynasty.It is a representative formula for supplementing Qi(气)and promoting blood circulation,and is widely used in diseases with Qi deficiency and blood stasis as the basic pathogenesis.Due to its good curative effect,this formula has been widely used in the treatment of a variety of clinical diseases.Meanwhile,as a commonly used prescription in cardiovascular,neurology and endocrinology departments,this prescription lacks clinical data summary in some orthopedic diseases.In recent years,this prescription has been gradually applied in orthopedic clinic,and has better efficacy and lower adverse reactions for some orthopedic diseases,so it has gradually attracted the attention of scholars and clinicians.This paper mainly introduces the application of Buyang Huanwu decoction in orthopedic diseases,discusses its composition and function,function mechanism of seven traditional Chinese medicines in orthopedic diseases,modern pharmacological research on the prevention and treatment of various orthopedic diseases,etc.At the same time,this paper further summarizes the good clinical efficacy of Buyang Huanwu decoction in some orthopedic diseases,especially in spinal diseases,arthritis,rotator cuff injury after surgery,joint replacement and DVT prevention after surgery.From the aspects of the basic theoretical basis of traditional Chinese medicine,the evidence of modern pharmacological research and the evaluation data of clinical efficacy,the important role and curative effect of Buyang Huanwu decoction on orthopedics dominant diseases are summarized,so as to further develop and utilize its efficacy and explore its application value in orthopedics.
