摘要
目的:研究螺旋下肢动脉CT血管成像CTA血管成像用于诊断下肢动脉硬化闭塞症的价值。方法:以2021年1月~2022年5月在本院接受治疗的下肢动脉硬化闭塞症患者51例为研究对象,所有患者均接受下肢动脉CT血管成像(CTA)检查以及数字减影血管造影(DSA)检查,以DSA检查结果为金标准。对比CTA在腓动脉、胫后动脉、胫前动脉、股浅动脉、腘动脉、股动脉、骼外动脉、骼总动脉8节段评定其诊断准确率、特异度与敏感度,同时分析其阳性、阴性诊断预测值情况。结果:51例患者经两种诊断方式诊断后,不同诊断下肢动脉节段图像各400个。其中两种诊断结果一致的阶段有385个,一致率为96.25%。其他15个不相符的节段中,7个节段为低估,8个节段为高估。经Kappa检验分析得出,Kappa值=0.969,证实两种诊断结果具有较高的一致性;以DSA诊断结果为金标准,得出CTA在腓动脉、胫后动脉、胫前动脉、股浅动脉、腘动脉、股动脉、骼外动脉、骼总动脉8节段的诊断准确率、特异度与敏感度,同时分析其阳性、阴性诊断预测值中均高于90%,证实了CTA诊断在该疾病中具有较高的诊断效能。结论:螺旋下肢动脉CT血管成像CTA血管成像用于诊断下肢动脉硬化闭塞症诊断中具有较高的应用价值,能够根据成像情况为后续临床治疗提供辅助依据,临床应用价值显著。
Objective:To study the value of spiral CT angiography in the diagnosis of arteriosclerosis obliterans of lower limbs.Methods:51 patients with arteriosclerosis obliterans of the lower extremities who were treated in our hospital from January 2021 to May 2022 were selected as the research objects.All patients underwent CT angiography(CTA)and digital subtraction angiography(DSA)of the arteries of the lower extremities.The results of DSA were taken as the gold standard.The diagnostic accuracy,specificity and sensitivity of CTA in 8 segments of peroneal artery,posterior tibial artery,anterior tibial artery,superficial femoral artery,popliteal artery,femoral artery,external iliac artery and common iliac artery were compared,and their positive and negative predictive values were analyzed.Results:After 51 patients were diagnosed by two diagnostic methods,there were 400 images of lower limb artery segments with different diagnosis.There were 385 stages in which the two diagnostic results were consistent,and the consistent rate was 96.25%.Among the other 15 inconsistent segments,7 segments were underestimated and 8 segments were overestimated.The Kappa test showed that the Kappa value=0.969,which confirmed the high consistency of the two diagnostic results;Taking the DSA diagnosis results as the gold standard,the diagnostic accuracy,specificity and sensitivity of CTA in 8 segments of peroneal artery,posterior tibial artery,anterior tibial artery,superficial femoral artery,popliteal artery,femoral artery,external iliac artery and common iliac artery were obtained.At the same time,the positive and negative predictive values of CTA were higher than 90%,which confirmed that CTA diagnosis had high diagnostic efficacy in this disease.Conclusion:Spiral CT angiography of lower extremity artery CTA angiography has high application value in the diagnosis of lower extremity arteriosclerosis obliterans.It can provide auxiliary basis for follow-up clinical treatment according to the imaging situation,and has significant clinical application value.
