摘要
目的探究合并粒细胞减少症对^(131)I治疗Graves甲亢的影响以及粒细胞减少症的治疗效果与甲亢的关系。方法选取2016年7月至2021年7月于我科行^(131)I治疗的180例Graves甲亢患者为研究对象,根据有无粒细胞减少症分为两组,其中伴有粒细胞减少组患者80例,无粒细胞减少组患者100例。比较两组患者^(131)I治疗甲亢的效果,分析粒细胞减少症的疗效与甲亢的关系。结果(1)^(131)I治疗前对两组患者的基线资料进行比较,粒细胞减少组的白细胞计数、中性粒细胞绝对值均低于粒细胞正常组,两者之间差异有统计学意义,但在性别、年龄、甲状腺质量、最高摄碘率、治疗剂量、甲状腺激素水平以及TRAb方面差异均无统计学意义,两组具有可比性。(2)^(131)I治疗后随访12月,复查甲功发现,两组患者FT_(3)、FT4均明显减低,TSH升高,甲状腺功能逐渐恢复至正常,粒细胞减少组与粒细胞正常组的甲亢治愈例数分别为66例、87例,差异无统计学意义(P>0.05),合并粒细胞减少症并不会影响^(131)I对Graves甲亢的治疗效果。(3)根据甲亢治愈情况,将Graves甲亢伴粒细胞减少症患者分为甲亢治愈组和未愈组,比较粒细胞减少症的治疗情况,甲亢治愈组的粒细胞减少症治愈率为91.3%,显著高于甲亢未愈组的58.8%,粒细胞减少症的疗效与甲亢有关。结论^(131)I对Graves甲亢伴粒细胞减少症患者同样具有很好的治疗效果,随着甲亢的治愈,粒细胞减少症的治愈率也会提高。
Objective To explore effects of combined granulocytopenia on^(131)I treatment of Graves hyperthyroidism and the relationship between the therapeutic effect of granulocytopenia and hyperthyroidism.Methods 180 Graves hyperthyroidism patients who underwent^(131)I treatment in our department from July,2016 to July,2021 were selected as the research subjects.These patients were divided into two groups according to the presence or absence of granulocytopenia,including 80 patients in the group with granulocytopenia and 100 patients in the group without granulocytopenia.The effect of^(131)I on hyperthyroidism was compared between the two groups,and the relationship between the effect of granulocytopenia and hyperthyroidism was analyzed.Results(1)Before^(131)I treatment,the baseline data of the two groups were compared.The white blood cell count and the absolute value of neutrophils in the granulocytopenia group were significantly lower than those in the normal granulocyte group.But there were no differences in gender,age,thyroid quality,maximum iodine uptake rate,therapeutic dose,thyroid hormones level and TRAb,for which the two groups were comparable.(2)Following up for 12 months after^(131)I treatment,thyroid function reexamination showed that FT_(3)and FT 4 in the two groups were significantly reduced,TSH increased,and thyroid function gradually returned to normal.The numbers of hyperthyroidism cured in the granulocytopenia group and the normal granulocyte group was 66 and 87,respectively,with no statistical difference(P>0.05).The combination of granulocytopenia did not affect the therapeutic effect of^(131)I on Graves hyperthyroidism.(3)According to the cure of hyperthyroidism,Graves’hyperthyroidism patients with granulocytopenia were divided into two groups:hyperthyroidism cure group and non-cure group.When comparing the treatment of granulocytopenia,the cure rate of granulocytopenia in hyperthyroidism cure group was 91.3%,significantly higher than 58.8%in hyperthyroidism non-cure group.The curative effect of granulocytopenia was related to hyperthyroidism.Conclusion^(131)I also has a good therapeutic effect on Graves hyperthyroidism with granulocytopenia.With the cure of hyperthyroidism,the cure rate of granulocytopenia will also increase.
