膀胱癌患者围术期营养管理的证据总结

Summary of evidence on perioperative nutritional management of patients with bladder cancer

摘要 目的检索、评价并整合膀胱癌患者围术期营养管理的相关证据,并汇总最佳证据,为临床实践提供参考。方法通过检索BMJ Best Practice、UpToDate、加拿大安大略注册护士协会循证护理指南网、苏格兰学院间指南协作网、英国国家卫生与临床优化研究所网站、欧洲临床营养和代谢学会网站、Cochrane Library、PubMed、CINAHL、Springer Link、Elsevier Science Direct、OVID、医脉通、中国知网、万方、维普、中国生物医学文献数据库等关于营养管理的证据,包括指南、系统评价、证据总结、最佳实践及专家共识,检索时限为建库至2022年12月31日。使用临床指南研究与评价系统II评价指南的方法学质量;其他类型的研究分别使用乔安娜·布里格斯研究所循证卫生保健中心数据库(2016版)相应的评价标准进行评估。结果共纳入11篇文献,通过专家会议最终确定了4个主题,分别为营养筛查与评估、营养支持、肠道管理、健康教育,共24条最佳证据。结论研究运用循证方法获得了关于膀胱癌患者围术期营养管理的最佳证据,为规范化这类患者的营养护理提供了可靠的科学支持。医护人员在临床实践中,应根据所在科室的临床环境,充分结合患者病情,考虑患者意愿,有针对性进行证据转化和应用,改善患者临床结局,提高患者生活质量和护理质量。 Objective To retrieve,evaluate,and integrate the evidence on perioperative nutritional management in patients with bladder cancer,and summarize the optimal evidence to inform clinical practice.Methods Evidence on nutritional management,including guidelines,systematic reviews,evidence summaries,best practices,and expert consensus,was retrieved from BMJ Best Practice,UpToDate,Registered Nurses'Association of Ontario Evidence-Based Nursing Care Guidelines,Scottish Intercollegiate Guidelines Network,National Institute for Health and Care Excellence,European Society for Clinical Nutrition and Metabolism,Cochrane Library,PubMed,CINAHL,Springer Link,Elsevier Science Direct,OVID,Medlinker,China National Knowledge Infrastructure,Wanfang Data,VIP Database for Chinese Technical Periodicals,and China National Biomedical Literature Database.Records published from the establishment of the databases until December 31,2022 were included.The methodological quality of the guidelines was assessed using the Appraisal of Guidelines for Research and Evaluation II tool,and other types of studies were evaluated using the corresponding criteria in the Joanna Briggs Institute Database of Systematic Reviews and Implementation Reports(2016 edition).Result A total of 11 articles were included,and four themes were identified based on expert panel review,namely nutritional screening and assessment,nutritional support,intestinal management,and health education,on which a total of 24 pieces of best evidence were summarized.Conclusions This study used an evidence-based approach to generate the best evidence on perioperative nutritional management in patients with bladder cancer,providing solid rationales for standardized nutritional care in these patients.In clinical practice,healthcare professionals should take in to account the clinical environment,patient conditions,and patient preferences,and translat the evidence to tailored management plan to improve patients'clinical outcomes and quality of life.
